Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed red build-up on the can opener blade.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Observed rolls of paper towels for hand drying stored on the floor in the bathroom. Store hand towels at least six inches off of the floor to prevent contamination from the floor, or while mopping. If these items are stored in the restroom, they shall be protected from airborne contaminants as well by storing in a cabinet or leaving them in their original boxes.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed. Observed a broken pan in use. Discard/replace all broken/cracked pans, lids, etc... as they are no longer smooth and easily cleanable.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed the wire rack in the walk in cooler in need of cleaning due to a build up of food debris. Observed the drying racks being sticky/tacky and in need of cleaning. Once clean, maintain it clean to sight and touch.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the seal at the back of the three compartment sink being black, with a mold-like substance. Clean the seal, or scrape it out and reseal the sink.

901:3-4-16(C) / Level one Certification in Food Protection
RFE does not comply with Level One Certification requirements. Observed no employee present with Person in Charge training in food protection. When the employee with ODH manager certification in food protection is not present in the facility, an employee with Person in Charge training shall be present, per Ohio Food Code.
