[?] A summary of the violations found during the inspection are listed below.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. Observed acoustic ceiling tiles in the back kitchen area.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-09(B) / Facility layout and equipement specifications - acceptable equipment
Repeat Observed the use of equipment that has not been approved. Observed dishes in a homestyle dish washer. This is not a commercial grade dish machine that can be used in a commercial facility. Discontinue the use of the dish machine. All dishes and equipment must be washed-rinsed-sanitzed in the three compartment sink. The dish washer must be removed from the facility.
Food equipment that is acceptable for use in a FSO or RFE shall be approved as specified under paragraph (KK) of rule 3717-1-04.1 of the Administrative Code.

Note) Observed a homestyle freezer being used. Once this does not work properly it must be replaced with a commercial grade freezer.
