[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. The quat sanitizer in the espresso machine wipe cloth container was low at time of inspection. Maintain quat sanitizer at 200ppm to safely sanitize food contact surfaces.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed black build up inside the ice machine. Thoroughly clean and sanitize the interior of the ice machine.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Improper storage of food items. Observed a box of food stored on the floor of the walk-in cooler. Store all food at least six inches off the floor to prevent contamination.

3717-1-03.2(M) / Wiping cloths - use limitation.
Improper use and/or maintenance of wiping cloths. Observed a wet wiping towel under the espresso machine. Store all wet towels in a bucket of sanitizer to control the growth of bacteria.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Observed stir spoons stored in a container of 122F water. The spoons shall be kept in a clean, dry container or in water held at 135F or higher.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed the gasket on a small cooler is torn. The PIC stated that the entire door will be replaced soon.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed damaged floor tiles by the ice machine, by the back door and inside the mop room. Repair the floor so that all portions of it are smooth, non-absorbent and easily cleanable.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed black build up on the wall behind the cabinet where the hand sink is located. Thoroughly clean the wall to completely remove the black build up.
