| consultation with the person in charge at the facility in regards to the WATER BOIL Advisory that was issued by the City of Toledo Department of Public Utilities (Division of Water Distribution) effective as of Wednesday 2/4/16 until Saturday 2/07/16 at 5:00 pm unless notified otherwise. At time of inspection, observed the facility boiling water from the tab water and using it for food preparation and sanitizing of food utensils and equipment. The facility is not using any equipment connected to the city water due to the above Advisory. All hand washing sinks are stocked with soap and paper towel (hot water available in insulated thermo cooler). Make sure hands are properly washed to prevent contamination.
