[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Handwashing sink is being used for purposes other than handwashing. Observed a block of ice in the hand sink at the far end of the cook line. Hand sinks may only be used for hand washing, and no other purpose.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Cheese is 44F and burgers are 43F in the left side of the drawer cooler at inspection. Manager found a piece of plastic that was blocking air flow to that side of the cooler. All other items in the right side were at proper temperature below 41F.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service or single-use articles. Observed a single use cup in the container of thousand island dressing on the expo line.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed excessive frost build up around the door of the walk in freezer. Observed dust build up on the wall mounted Imperial grill. Observed build up on the shelf beneath the french fry warmer.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed a leak coming from the ceiling over the clean dish table of the dish machine. Manager stated that they have had issues with leaks when there is precipitation/snow. Manager has contacted the mall for repairs. This area may not be used for dishes until the leak is repaired.

VI - TCS Food: TCS foods were not being held at the proper temperature. The drawer cooler had a piece of plastic blocking air flow to one side. This was corrected at inspection.
VI - P - TCS Food: Observed ready to eat TCS foods being properly date marked, and discarded when required.
VII - P - Protection from Contamination: Observed raw animal foods separated by type during storage, preparation, holding, and display.
VIII - P - Consumer Advisory: Observed a consumer advisory on the menu for animal foods that are served raw, undercooked or not otherwise processed to eliminate pathogens.
X - P - Chemical: Toxic materials are properly identified and stored.
