[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Observed the quaternary ammonium sanitizer in the three bay sink exceeding 500 ppm. The water was drained and water added to achieved a concentration of 300 ppm.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed decanters of salsa at the server station holding at 51 degrees. There was not enough ice around the containers to maintain the salsa below 41 degrees. An employee added ice and the salsa temped at 39 degrees within 15 minutes. Observed cut heads of lettuce and cut tomatoes in the make table, stored on pans above the fill line. PIC discarded these items. Cease storing food in the make table above the fill lines. Observed all items in the left side make table holding between 49 and 53 degrees. PIC discarded all TCS food items in the table and called to have the table serviced.

3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Working containers of poisonous or toxic materials not properly labeled. Observed an unlabeled chemical spray bottle with a yellow liquid in it. Label all chemical spray bottles so accidental misuse or poisoning does not occur.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Repeat Insufficient air gap between the flood rim and the water supply inlet. Observed no air gap present at the bar ice bin or the Pepsi ice bin. Install air gaps for backflow prevention at all ice bins.

3717-1-04.2(A) / Equipment - cooling, heating, and holding capacities.
Inadequate equipment available for cooling, heating or holding food. Observed the left side make table unable to maintain food below 41 degrees. The table was iced down so the facility could make it through lunch service and a call was made to get the table serviced.

3717-1-03.4(C) / Thawing - temperature and time control.
Corrected During Inspection TCS foods not properly thawed. Observed a package of ROP fish being thawed without being removed from the packaging, per instructions on the package. PIC discarded the improperly thawed fish.

3717-1-03.2(G) / Washing fruits and vegetables.
Corrected During Inspection Raw fruit and/or vegetables not washed or washed improperly. Observed a cut avocado in the right side make table with the sticker still on it. Remove stickers and wash all produce prior to cutting as contaminants on the skin can be pushed into the meat of the produce being cut. The cut avocado was discarded.

3717-1-03.2(K) / In-use utensils - between-use storage.
Corrected During Inspection In-use utensils improperly stored. Observed the bar ice scoop stored with the handle touching the ice. If the ice scoop is to be stored in the ice, it must be done in a manner with which the ice does not contact the scoop handle.

3717-1-04.4(B) / Cutting surfaces.
Repeat Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed cutting boards with excessive staining and scarring, in need of being resurfaced or replaced.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the pipe under the three compartment sink leaking. Repair the pipe so the sinks do not leak.
