[?] A summary of the violations found during the inspection are listed below.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed a build-up on the ice machine slide. Clean and sanitize the slide on the inside of the ice machine. Ice is a RTE food and must be protected from contamination. Corrective Action: The ice machine will be cleaned and sanitized tonight and all of the ice will be removed prior to cleaning. Clean the inside of the ice machine door while you are cleaning the slide.

3717-1-07.2 / Poisonous or toxic materials: storage and display - separation
Critical Improper display of poisonous or toxic materials offered for retail sale. On the grocery isle chemicals and beauty products must be stored below any single service items. Corrective Action: Storage order will be switched around tonight on the shelves.

3717-1-03.5(C)(1) / Food labels - packaged food
A food packaged in the facility not labeled. Observed bagged ice offered for sale in the facility that was packaged in the facility and was not labeled. Bagged ice must be labeled with the following information: Name and address of the store, and the weight of the ice in the bag.

3717-1-03.5(A) / Standards of identity
Packaged food labels are not in compliance with standards of identity. Observed bags of frozen onion rings and French fries without the manufacturers label. Provide the manufacturers label on these products.

3717-1-06.1(K) / Insect control devices - design and installation.
Repeat Corrected During Inspection Improper use or placement of insect control devices. Observed a yard bug light in use in the meat room. Remove the bug light. Replace the bug light with an approved commercial light that prevents bugs or bug parts from leaving the light/unit. Light was removed at the time of the inspection.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed the wall mounted exhaust fan on the wall of the pop stock room not screened. Screen the exhaust fan on the inside of the room with 16 mesh to 1 inch screening. Screen the fan to prevent pests from entering the facility. Observed the front door of the facility open and not protected. The front door must be maintained closed as to prevent the entrance of pests.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed raw chicken wings in the meat display case stacked too high for the pans they were in. Raw chicken wings cannot overflow the pans they are being displayed in. Raw chicken wings are not to make contact with the meat display case so as to prevent contamination.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed food being stored on the floor of the walk-in pop cooler and on the floor of the walk-in freezer. Store all cases of food at least 6" up off the floor to prevent contamination.

3717-1-03.2(M) / Wiping cloths - use limitation.
Improper use and/or maintenance of wiping cloths. Observed wet wiping cloths on the prep table. Store all wet wiping cloths in sanitizer solution at the correct strength between uses. Wet wiping cloths left out can grow and harbor bacteria.

3717-1-04.8(F) / Storage - prohibitions - single-use/single-service articles.
Repeat Facility is working on cleaning out the basement. Remove all of the boxed single service items from the basement as they cannot be stored in the basement.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed the prep table in the 3 compartment sink room no longer smooth and easily cleanable. Replace the prep table. Owner stated that a new prep table would be purchased. Observed broken glass on the doors of the meat display case. The glass must be replaced so that it is smooth and easily cleanable. Owner states that the glass is on order and special order due to the age of the cooler. Observed a light in the meat display case held up inside the unit with plastic tie. Repair the light holder in the unit. Must be smooth and easily cleanable.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed the prep table that the meat grinder sits on in the walk-in cooler in need of cleaning. Thoroughly clean and sanitize the prep table.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed a leak on the drain of the handsink located in the 3 compartment sink room.

3717-1-05.1(B)(1) / Approved plumbing system.
Plumbing system does not comply with the Ohio plumbing code. Observed the drain line from the ice machine going up in elevation to the floor drain. The ice machine drain line must be sloped to drain to the floor drain with the correct amount of fall per the Ohio plumbing code. Fix the fall on the drain line so that the line properly drains to the floor drain.

3717-1-05.4(B) / Outdoor storage surface.
Outdoor refuse storage surface not properly constructed and sloped to drain. Observed the trash dumpster and the grease dumpster on the grass/dirt. The dumpsters must be located on the asphalt. Have the dumpsters relocated so they are on the asphalt.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed the wall at the entrance to the meat display case/area in need of repair. Mortar the wall and then paint it. Observed broken and damaged ceiling tiles in the meat counter area. Replace these tiles with vinyl faced ceiling tiles.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed the wall to the pop storage room and to the back storage area not finished to be smooth and easily cleanable. Finish the walls so they are smooth and easily cleanable.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed the floor under the isle shelves in need of cleaning. Observed the floor in the pop storage room in need of cleaning. Observed the floor in the back stock room and in the 3 compartment sink room in need of degreasing and cleaning. Observed the room where the flour is stored in need of organizing and cleaning. Observed the fan guards in the walk-in cooler in need of cleaning.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Floor and wall junctures not properly coved or closed and/or floor drains not provided. Observed the coving/trim missing from the base of the walk-in freezer. Supply the missing piece of trim. Observed the coving behind the fryers having fallen off. Reattach the coving behind the fryers.

3717-1-06.1(I) / Light bulbs - protective shielding.
No protective shielding on lights. Provide a plastic coated bulb on the light in the pop stock room.

Met with Ishmail today during the inspection. Facility is making good progress and working on the facility. Keep up the good work!!

Consulted: Do not use cardboard on the cooler/freezer shelves as it's not smooth and cleanable.
Note: Facility has been cleaning out the back stock room and it is really improving!! Great job, keep up the good work!
Consulted: Store al boxes of single service items up off the floor of the back stock room.
Note: Facility has been painting the ceiling throughout and added a ceiling to the area leading to the basement. Looks good, keep up the great work!
Consulted: Do not turn off the hot water at the valve under the handsinks. Repair the leak issue on the faucets. Hot water must be turned on and off at the faucet knobs.
Consulted: All TCS RTE foods held longer than 24 hours must be properly date marked in the prep top cooler. At the end of the night date mark anything that you'll be holding over.
Note: The Minute Maid cooler is going to be replaced, a new cooler is on order. Minute Maid cooler has electrical issues and the inside is not smooth and easily cleanable. Supply the health department with the make and model of the new cooler or any other new pieces of equipment you order. We need to pre-approve all new equipment to make sure it meets all requirements.
Note: Facility added new outdoor lighting on the building.
