[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(C)(11) / PIC: duties - ensure employees are proplery sanitizing cleaned equipment and utensils
Critical PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters. Observed an employee scrubbing a pan in sanitizing water, rinsing it, and then placing it on a stack of clean plans. The employee was told to properly wash, rinse, and sanitize the pan. Ensure all dishwashing staff are properly trained in the proper procedures for warewashing and the procedures are utilized.

3717-1-02.4(B)(1) / PIC: Demonstration of Knowledge - No Critical Violations
Critical PIC unable to demonstrate knowledge by having no critical violations.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed utensils and strainers by the chemicals dispenser getting sprayed on by the chemical dispenser while the wash cycle was going. Remove the utensils and strainers, wash, rinse, and sanitizer them, and stored them in a location where they will not be contaminated.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed pre-cooked turkey sausage patties holding at 46 degrees in the prep top cooler. The sausage was stacked above the fill line and the cooler could not maintain them below 41 degrees. PIC had all of the out of temperature sausage discarded. Cease filling pans above the fill line.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Repeat Insufficient air gap between the flood rim and the water supply inlet. Observed no air gap at the service line ice bin. Install an air gap at the ice bin for backflow prevention.

3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Handwashing sink water below 100°F. Observed insufficient hot water/water flow at the restroom handwashing sinks. Repair the sinks so proper water flow is available to allow customers to properly wash their hands.

3717-1-04.1(Y) / Temperature measuring devices.
Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Observed no thermometer in the skim milk cooler.

3717-1-06.1(M) / Outer openings - protected.
Corrected During Inspection Outer opening not protected. Observed the back door being propped open at time of inspection. Cease propping the door open as it allows rodents, insects, and/or other pests access to the facility.

3717-1-02.3(C) / Hair restraints - effectiveness.
Repeat Food employee(s) not wearing a hair restraint. Observed employees working without properly restrained hair, having pony tails hanging in front and/or hair hanging out in front of their ears. (C) Hair restraints - effectiveness. Food employees shall effectively restrain hair by wearing hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food; clean equipment, utensils, or linens; or unwrapped single-service or single-use articles. This provision does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Observed single service cups stored next to the handwashing sink and having splash stains on the box. Store single service items in a location where they cannot be contaminated by the handwashing sink, or install a splash guard to protect the items.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed the drying racks being sticky to the touch. Clean the drying racks and maintain them clean to sight and touch.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the P&G Professional Chemical Dispenser spraying water when the detergent is being dispensed. Repair the chemical dispenser so it does not spray water, possibly contaminating food contact surfaces in the area. Observed the mop sink leaking from the bell. Repair the leak.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed the walk in cooler floor in need of cleaning. Observed cleaning needed under all of the racks, under the ice machine, under the three compartment sink, Observed the wall behind the three compartment sink in need of cleaning.

3701-21-25(I) / Person in charge certification in food protection
FSO did not have a person in charge per shift with the person in charge certification in food protection. Observed no employee present with person in charge training.
