[?] A summary of the violations found during the inspection are listed below.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed that he slicer had a build-up of food debris. The slicer is to be maintained clean to the site and the touch. Slicer is to be completely broke down and properly cleaned and sanitized at least once every 4 hours. If contamination occurs the slicer is to be cleaned more often. Corrective Action: Slicer was placed out of service until it could be completely broken down and properly washed, rinsed, and sanitized.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed the facility re-use raw chicken boxes for the storage of food, equipment, and linens. Raw chicken boxes are to never be re-used for any purpose. Once empty of the original chicken the box is required to be discarded immediately. Corrective Action: All foods that were in direct contact with the boxes were discarded at the time of the inspection. The bagged linens were moved. All of the chicken boxes were thrown out at the time of the inspection

3717-1-03.3(H) / Reheating for hot holding.
Critical Improper reheating of food for hot holding. Observed an open can of cheese sauce in the hot holding cabinet at an internal temperature of 77 degrees. The can appeared to have been in use on a previous day. All TCS foods that have been properly cooled are to be reheated to 165 degrees rapidly on a conventional piece of cooking equipment (stove or oven) with-in 2 hours or less. Hot holding cabinets are not to be used for reheat or heating, only for hot holding foods that have been properly heated. Corrective Action: The can of cheese sauce was discarded at the time of the inspection.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Observed in house produced hot sauce at room temperature. All house made sauces must be stored in under refrigeration at 41 degrees or less to prevent the growth of pathogens. Corrective Action: Two containers of the sauce were discarded at the time of the inspection.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Observed raw shell eggs and cheese slices sitting out. Eggs were at 59 degrees. All TCS foods must be maintained at 41 degrees or less and the raw eggs must be maintained at an ambient air temperature of 45 degrees or less to prevent the growth of pathogens. Corrective Action: The eggs and cheese slices were discarded at the time of the inspection.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed multiple lunch meats, cooked potatoes, turkey bacon, Tazaki Sauce all without date marking. All TCS RTE foods must be properly date marked to reflect the seven days in which they must be sold, consumed, or discarded. Corrective Action: The items that were lacking a date mark were discarded at the time of the inspection.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed that the back exterior door does not seal tight. Provide a door sweep on the back exterior door. No light from the outside is to be visible on the interior. Observed wall mounted exhaust fans not properly screened to prevent the entrance of pests. Screen all outer openings with 16 mesh or more per square inch.

3717-1-06.1(N) / Exterior walls and roofs - protective barrier.
Repeat Perimeter walls and/or roof do not provide adequate protection from weather, insects, rodents, animals, etc. Observed that the bricks on the exterior of the building are falling off. Have the exterior walls/bricks repaired. Owner stated that they were in contact with multiple contractors to have the issue addressed. Walls must be in good repair and provide protection from the outside.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment not approved by a recognized testing agency. Observed the facility with a homestyle toaster and coffee pot. All equipment must be commercial NSF approved. Remove the homestyle equipment.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed a condensate leak in the interior of the walk-in cooler. Have the walk-in cooler repaired so that it no longer leaks condensate into the interior of the unit. Service company was contacted.

3717-1-04.4(D) / Warewashing equipment - cleaning frequency.
Warewashing equipment and/or components not cleaned at required frequency. Observed that all 3 compartments of the 3 compartment sink were dirty. The 3 compartment sink is to be maintained clean to the sight and touch. When the facility is open and operating the 3 compartment sink is to be set up and properly operated.

3717-1-05.4(H) / Toilet room receptacle - covered.
No covered receptacle in rest-room. Provide a covered trash can in the rest-room.

3717-1-05.4(B) / Outdoor storage surface.
Outdoor refuse storage surface not properly constructed and sloped to drain. Observed the grease dumpsters on the grass/dirt. The grease dumpsters must be on the asphalt surface of the driveway/parking area.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Finish the walls in the rest-room and then paint them. Walls must be in good repair and finished to be smooth and easily cleanable.

3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. Observed acoustic ceiling tiles in the meat counter/deli counter/kitchen area. All of the ceiling tiles in this entire area must be vinyl faced, acoustic tiles are not permitted in these areas. Replace the acoustic tiles with vinyl faced tiles.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed the room behind the cash out counter to be in need of cleaning and organizing. Clean and organize this entire area and then maintain it clean.

Comments
Met with Ish and George during the inspection. Facility has done a lot of work and is continuing to work on the entire facility. Facility plans to cover all of the new wooden shelving in the back stock area with FRP.

Facility has applied for the Facade Grant through the City of Toledo.

The exterior bricks are falling off the building on the outside.

Facility has been working on a lot of issues on the interior, and continues to make progress.
