[?] A summary of the violations found during the inspection are listed below.

3717-1-03.1(L) / Package integrity - specifications for receiving
Critical Food package(s) received in poor condition. Observed a dented can of pineapple in the facility. Refuse, return, or discard cans dented on the top, bottom, or side seams of the can.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the can opener having food debris on the blade. Clean the blade of the can opener after each use so it is readily available for use. Observed the drying racks being sticky, thus making lids and dishes stored on them unclean due to contact. Immediately clean the racks so clean dishes can be stored on them without becoming contaminated.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Repeat Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Observed the quaternary ammonium sanitizer being dispensed at 500+ppm, which is above the required concentration of 200ppm to 400ppm. Repair the sanitizer dispenser so dishes may be sanitized properly.

3717-1-02.4(A)(2) / PIC - Manager Certification
Facility does not have an employee with manager certification in food protection.

3717-1-02.3(C) / Hair restraints - effectiveness.
Repeat Food employee(s) not wearing a hair restraint. Observed an employee working with food while wearing a visor. The top of the head needs to be covered and the hair properly restrained while food is being prepped or made.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the red sanitizer hose leaking while the water is turned on in the sink. Repair the hosing/connection so there is no leak.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed a broken ceiling tile in need of replacement. Observed a torn air curtain in the walk in cooler in need of being replaced or removed. Observed the floor threshold at the back door being cracked and broken, in need of repair.

901:3-4-02(G) / RFE must display license
RFE license not posted in the facility. Observed no license, for the 2024 - 2025 food year, in the facility.

901:3-4-16(C) / Person in Charge Certification in Food Protection
Repeat RFE does not comply with Person in Charge Certification requirements.

Failure to do so may result in a summons to the Toledo Lucas County Health Department to explain the facility's non-compliance, which is the first step towards revocation of the facility's food license.
