[?] A summary of the violations found during the inspection are listed below.

3717-1-03.1(A)(6) / Sources - safe handling instructions on raw meat and poultry
Critical No safe handling instructions on packaged raw meat. Observed mussels and catfish fillets that were not labeled at all offered for sale. These items must be properly labeled for retail sail including safe handling instructions.Corrective Action: All of these items were removed from sale at the time of the inspection.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Food not properly protected from contamination by separation, packaging, and segregation. Observed the facility re-using raw chicken boxes to store foods in other than raw chicken including but not limited to candy and cut onions. Foods shall be protected from contamination at all times while in the facility. Once empty of the original raw chicken that came in the box, the box must be discarded. Boxes cannot be washed, rinsed, and sanitized. Corrective Action: The raw chicken boxes and all foods that were stored in them were discarded at the time of this inspection.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed a mold like substance on the interior of the ice machine. Corrective Action: Discard all of the ice in the ice machine. Thoroughly clean and sanitize the entire interior of the ice machine. Maintain the ice machine clean to the sight and touch.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the mixer not clean to the sight and touch. A food build-up was noted on the underside of the head and on the components of the mixer. Corrective Action: Thoroughly clean and sanitize the entire mixer and then maintain it clean.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the slicer not clean to the sight and touch. Old and dried food debris was visible on the slicer base and blade. Corrective Action: Facility is to cease use of the slicer until it is completely broken down and washed, rinsed, and sanitized. Slicer is to be maintained in clean state at all times.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed facility re-using boxes that raw chicken came in originally to store candy, onions, beef, fish and corn on the cob. Boxes cannot be reused as they cannot be properly washed, rinsed, and sanitized. Corrective Action: Cease the re-use of any and all boxes in the facility. Provide sanitized food grade containers for the proper and safe storage of all foods in the facility. All foods in the direct contact with the boxes were discarded at the time of the inspection.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed the facility re-using cardboard boxes to bread chicken in. Corrective Action: All of the cardboard boxes were discarded at the time of the inspection. Provide sanitized food grade containers for breading raw chicken in. Do not re-use cardboard boxes for any purpose. Boxes are to be immediately discarded once empty of original contents.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Observed the beer walk-in cooler ambient air temperature at 41 degrees. Gyro sauce stored in the walk-in cooler had an internal temperature of 45.5 to 46 degrees. All TCS foods must be maintained at 41 degrees or less internally to prevent the growth of pathogens. Corrective Action: All of the Gyro sauce in the cooler was discarded at the time of the inspection. This included 5 cases of Gyro sauce. No other TCS foods were being stored in the walk-in cooler.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed multiple lunch meat loaves that were open and in use that did not have a date mark. All TCS RTE foods must be properly date marked to reflect the 7 days in which they must be sold, consumed, or discarded by. Corrective Action: All TCS RTE foods that were not date marked at time of this inspection were discarded.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Ready-to-eat, TCS food not properly discarded when required. Observed TCS RTE foods that were being held and offered for sale past the allowed 7 days. Corrective Action: All of the TCS RTE foods that were past the allowed 7 days were discarded at the time of the inspection.

3717-1-06.4(K) / Controlling pests.
Critical Repeat Presence of live insects, rodents, and other pests. Observed the facility storing all sorts of unnecessary items in the back stock room and in the mechanical room. The extensive storage of all sorts of items is creating a pest harborage issue. Corrective Action: Clean out the back stock room and the maintenance room. Clutter is preventing the proper maintenance and cleaning of the facility.

3717-1-03.5(B) / Honestly presented.
Food products not honestly presented. Observed multiple commercially packaged foods that were that were not labeled in any fashion and were being offered for sale. All foods and beverages must be properly labeled including the manufacturers label to be offered for retail sale. Including: French fries, onion rings, poppers, crab legs, mussels, cheese sticks, Capri Sun, etc. Stop offering for sale foods and beverages that are not properly labeled and/or are not labeled for individual retail sale.

3717-1-03.5(C)(1) / Food labels - packaged food
Repeat A food packaged in the facility not labeled. Observed the facility packaging: bagged ice, corn-on-the-cob, and catfish fillets/nuggets in house and offering them for retail sale. All foods must be labeled with the store name and address, name of the product, list of ingredients, and the net weight of the food. Cease offering for retail sale any and all foods and beverages that are not properly labeled.

3717-1-06.1(K) / Insect control devices - design and installation.
Improper use or placement of insect control devices. Observed fly strips located above food and food contact equipment. Insect control devices cannot be placed over food or equipment.

3717-1-06.1(N) / Exterior walls and roofs - protective barrier.
Perimeter walls and/or roof do not provide adequate protection from weather, insects, rodents, animals, etc. Observed several holes and openings to the outside throughout the facility. Screen all of the ventilation fans in the exterior walls, patch and seal all holes in the exterior walls, repair the exterior soffit as it is open and exposing the interior of the facility. The facility must be sealed and protected from the exterior, this is to prevent the entrance of pests.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Improper storage of food items. Observed food being stored on the floor of the walk-in freezer and throughout the facility. All foods must be stored at least 6" up off the floor to prevent contamination.

3717-1-04.8(E)(1) / Equipment, utensils, linens - storage.
Observed clean linens and wipe towels being stored in dirty shopping carts and in containers that were not clean. Store all clean towels and aprons in a sanitary area so that they do not become contaminated.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Repeat Re-use of single-service or single-use articles. Observed the facility washing and re-using single service tin pans. Disposable tin pans are to be used once and then discarded. Tin pans are not smooth and easily cleanable and are not to be reused.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Observed single service items being stored directly on the floor in the facility. Store all single service items up off the floor to protect them from contamination.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed the meat walk-in cooler leaking condensate into the interior of the walk-in cooler. Repair the cooler so that condensate does not leak and is not collected on the interior of the walk-in cooler.

Fixed equipment not properly sealed or spaced for cleaning. Observed the facility installed all new handwashing sinks. Sinks must be attached and sealed to the wall.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed the 2 door freezer behind the meat counter not functioning. Freezer has not functioned for an extended period of time. Repair the freezer or remove it from the facility.

Repeat Equipment components are not intact, tight or properly adjusted. Observed a shelf in the beer walk-in cooler held together with duct tape. Replace the shelf as it is no longer in good repair or smooth and easily cleanable.

Repeat Nonfood-contact surfaces constructed of unapproved materials. Observed cardboard on the shelves of the beer walk-in cooler. Remove all of the cardboard from all of the shelves in the facility as cardboard is not smooth and easily cleanable.

Equipment components are not intact, tight or properly adjusted. Observed the beer cooler not maintaining temperature. Repair the beer walk-in cooler.

Repeat Equipment components are not intact, tight or properly adjusted. Observed the deli wrap machine to be severely scored. Repair/replace the scored area so the machine is smooth and easily cleanable.

Repeat Non-durable equipment observed. Observed the facility using multiple walk-in coolers that no longer function for dry storage. Remove the walk-in coolers from the facility.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed the interior of the Coca-Cola cooler with a mold like substance on the surfaces. Clean the interior of the cooler and then maintain it clean.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed no hot running water at the mop sink. Provide hot running water at the mop sink at all times.

3717-1-05.4(D) / Receptacles.
Improper receptacles for refuse, recyclables, or returnables. Observed the facility using cardboard boxes for trash cans throughout the facility. Provide durable/washable trash cans throughout the facility including at each hand washing sink.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed the wall at the end of the meat counter not finished to be smooth and easily cleanable. Repair the wall and finish it to be smooth and easily cleanable.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed water pooling in the damaged areas of the meat walk-in cooler floor. Clean the floor and eliminate all standing water.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed the FRP panel on the ceiling in the flour room falling into the room. Repair the ceiling and reattach the FRP.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the corner between the hood and the side exterior wall in need of cleaning. Clean this area and then maintain it clean. (Below the exterior wall exhaust fan on the main cook line)

3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. Observed acoustic ceiling tiles above the meat counter and food preparation areas. Ceiling tiles in this area must be sealed and be smooth and easily cleanable. Replace the ceiling tiles with vinyl faced tiles.

Repeat Floor and wall junctures not properly coved or closed and/or floor drains not provided. Replace and reinstall all of the missing and loose cove base throughout the facility.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Repair all of the holes in the walls and ceiling of the back stock room.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. Observed the area/room behind the front check out counter to need a deep cleaning and organizing. Remove all unnecessary items and organize the entire area.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed missing and broken floor tiles in the facility. Replace all missing and broken floor tiles.

Repeat Physical facilities not maintained in good repair. Observed all of the paint/concrete sealer has peeled off of the floor in the 3 compartment sink room. Refinish the floor so that it is sealed and smooth and easily cleanable.

Floor has large areas that are depressed and are holding standing water. Repair or replace the floor in the meat walk-in cooler. Floor must be smooth and easily cleanable as well as sealed.

Repeat Unnecessary or nonfunctional items and /or litter on premises. Remove all homestyle equipment and unnecessary equipment from the facility.

3717-1-06(B) / Outdoor areas - surface characteristics
Improper outdoor surface characteristics. Observed that the grease dumpster was placed on the dirt/grass. Grease dumpsters must be located on the blacktop/concrete parking lot. Move the dumpsters back onto the paved parking lot area.

3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat No protective shielding on lights. Observed the light in the ceiling of the flour room not shielded. Shield the light or provide a coated bulb.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Light intensity less than ten foot candles in required areas. Observed 3 foot candles of light in the meat walk-in cooler, 10 foot candles are required. Replace the burnt out bulbs in the walk-in meat cooler.

RFE does not comply with Person in Charge Certification requirements. Provide at least one employee on-site during all hours of operation that is certified in food safety training.

At the time of the inspection no one in the facility was food safety trained at any level. Provide someone on-site at all times that is food safety trained and certified. Provide certificates on-site to verify training.
