3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed tuna salad in the make table holding at 44.5 degrees. Observed sliced turkey in the make table, and stock cooler by the make table, holding at 55.5 degrees. Observed almost all items in the stock cooler, by the make table, holding at 55.5 degrees. All items holding above 45 degrees were discarded.

3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects, rodents, and other pests. Observed black flying insects in the facility. Employ an exterminator, licensed to work in a kitchen, to eliminate the pests.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Repeat Insufficient air gap between the flood rim and the water supply inlet. Observed no air gap present at the prep sink. Observed no air gap present at the self service soda station and accompanying ice machine. Install air gaps at these locations for backflow prevention, per Ohio Food and Plumbing Codes.

Facility does not have an employee with manager certification in food protection. Education information provided.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Corrected During Inspection No written procedures for responding to vomiting or diarrheal events. Handout provided.

3717-1-04.2(A) / Equipment - cooling, heating, and holding capacities.
Inadequate equipment available for cooling, heating or holding food. Observed the stock cooler, by the make table, unable to hold food below 41 degrees. PIC moved all TCS items temping below 45 degrees, to the walk in cooler. PIC also moved all non-TCS items to the walk in cooler to avoid confusion. PIC stated no food would be held in the stock cooler until it is serviced by maintenance and it displays the capacity to hold water below 41 degrees.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Improper storage of food items. Observed a catering order in the walk in cooler, stored on the floor. PIC placed a small plastic rack in the walk in cooler and placed the food onto it.

3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) not wearing a hair restraint. Observed an employee working with food, with a beard longer than 1/4". Discussed the parameters for wearing hats, hair nets, and beard nets.

3717-1-02.2(G) / Jewelry - prohibition.
Food employees wearing jewelry on arms or hands during food preparation. Observed an employee making sandwiches while wearing a watch, (G) Jewelry - prohibition. (1) Except as provided in paragraphs (G)(2) and (G)(3) of this rule, while preparing food, food employees may not wear jewelry on their arms or hands. (2) A medical alert bracelet is permitted when a reasonable accommodation is made, such as wearing the bracelet high on the arm or secured in a manner that does not pose a risk to the food but provides emergency medical information if it is needed. (3) A plain ring such as a wedding band may be worn.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed. Observed the bottom metal trim, and the opposite side's siding, coming off of the self service soda station cabinet.

3717-1-04.2(J) / Cleaning and Sanitizers - Availability
Cleaning agents and/or sanitizers not available in the FSO or RFE. Observed no sanitizing bay, or bucket made at time of inspection. Food was being served during the inspection. Prior to food prep or service, a sanitizing bucket and/or sink bay shall be made to ensure food contact surfaces are wiped down prior to use.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed the racks in the walk in cooler having excessive food debris on them, and in need of cleaning. Observed food debris on the shelf below the prep table. Observed sticky cabinet doors below the self service soda station. Observed a pool of syrup in the cabinets under the self service soda station.

3717-1-05.1(Q) / Scheduling inspection and service for a water system device.
Water treatment device or backflow preventer not inspected/serviced with documentation as required. Observed the water filter, for the self service soda station, being dated for needing changed on 5/20/2015.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the walk in cooler and walk in freezer floors in need of cleaning.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed the light in the walk in cooler being non-functional and in need of repair. Observed the corner, going into the back storage area, being damaged and in need of repair, then potentially shielded from future damage via a hardened plate.

Upon entering the facility, the PIC was observed wearing Beats headphones around his neck.
