[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Repeat TCS foods not being cold held at the proper temperature. Observed flavored cream cheeses holding at 44.5 and 45.5 degrees. Observed pasteurized processed cheese slices holding at 49.5 degrees. Cheese was discarded until slices holding below 41 degrees were found. PIC stated she did not know why the items were out of temp. All other items were holding at 40 degrees or less.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Repeat Insufficient air gap between the flood rim and the water supply inlet. Observed no air gap at soda station ice bin by the drive through window.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection. Observed no employee with ODH certification in food protection dedicated to the facility.

3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Employee eating, drinking, or using tobacco in non-designated area. Observed employees drinks in a food prep area. PIC moved the drinks.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Corrected During Inspection No towels or drying device at the handwashing sink(s). Observed the paper towel dispenser by the front counter no effectively dispensing paper towels. PIC changed the batteries and the dispenser began working normally.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Food not protected from contamination. Observed a rack of donuts in the front counter area uncovered. If donut trays are on a rack, it shall be covered and efforts to protect the donuts from contamination shall be made.

3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) not wearing a hair restraint. Observed employees wearing visors, but not hair nets to cover the top of the head and effectively restrain hair. If a visor is being worn, and hair net shall be worn underneath to cover the top of the head and effectively restrain employee hair.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Observed coffee filters stored out in the open, exposed to potential contamination. Keep the filters in a plastic bag or closed container to prevent potential contamination from airborne particulates.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed. Observed the neutral base dispenser leaking product out of the sides and onto the counter. Clean the counter, maintain it clean, and repair the machine. If it cannot be repaired, replace or remove it.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed cleaning needed under warewashing and under the right side racks in the walk in cooler. Observed ceiling tiles in need of cleaning.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed cove base molding in need of being replaced/resealed under warewashing. Observed the light fixture by the oven area being nonfunctional.

3717-1-06.3 (C)(1) / Employee accommodations - designated areas
Employees breakroom improperly located. Observed a table and chairs placed by the dry goods rack in front of the walk in cooler. PIC stated it was where employees took breaks and stored their personal items. Per the original plan review letter, there is no employee break room, nor area for storing employee personal items in the food prep/service areas.

Observed the hot holding timers not in use. PIC stated she did not know how to operate them. Food in hot holding was holding above 135 degrees.

PIC did not know how to utilize the irreversible warewashing thermometer. Education provided.
