[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Corrected During Inspection Handwashing sink not accessible. Observed the front counter handwashing sink blocked by a bulk coffee container. PIC moved the container so the handwashing sink was accessible.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed whipped topping in a bag, partially submerged in ice, holding at 70 degrees. PIC discarded the bag and stated the practice would no longer be used in the facility. Observed the front make table holding food items between 50 and 51 degrees. PIC discarded all items within the make table and called to have the table serviced.

3717-1-06.4(K) / Controlling pests.
Critical Repeat Presence of live insects, rodents, and other pests. Observed several gnats in the interior of the facility, by the back door and the warewashing area.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed a gap allowing light in at the bottom of the back receiving door. Install a wire sweep to eliminate the gap.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Food not protected from contamination. Observed boxes of fruit smoothie mix, in the walk in cooler, with food purple food debris on them. Clean all up all spills when they occur so food items/containers are not potentially contaminated.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed all beverage dispensers in need of detailed cleaning.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the warewashing machine not functioning properly due to replacing the heat booster. PIC stated it should be repaired within 2 weeks and the three compartment sink is being used to clean dishes and utensils.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Improperly cleaned storage area for refuse, recyclables, or returnables. Observed excessive clutter, leaves, and junk in the dumpster area.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the cove base molding in the walk in in need of repair. Observed the seal behind the three compartment sink in need of being scraped out and replaced due to accumulation of grime.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed both walk in floors in need of cleaning due to food debris in the back and corners. Observed sweeping and mopping needed in back by the office and mop sink. Observed ceiling air vents in need of dusting. Observed cleaning needed under the ice machine, under warewashing, and under the three compartment sink.
