Critical Repeat Food not properly protected from contamination by separation, packaging, and segregation. Observed containers of uncovered food such as noodles, chicken, sauce, in the three door cooler. All containers of food shall be covered during storage to protect from contamination. Corrective Action: Place lids on foods during storage.

Critical Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed foods in the 3 door cooler such as noodles, chicken, seafood, rice, without date marks. Refrigerated, ready-to-eat, time/temperature controlled for safety food prepared and held in a food service operation or in a retail food establishment for more than twenty-four hours is to be clearly marked to indicate the date or day by which the food is to be consumed on the premises, sold, or discarded when held at a temperature of forty-one degrees Fahrenheit (five degrees Celsius) or less for a maximum of seven days. Observed home-use container storing salad Observed Ninja blender labeled "home-use only" Equipment shall be approved for commercial use by a recognized testing agency. Remove home-use equipment from the facility and provide commercially approved equipment.

Nonfood-contact surfaces constructed of unapproved materials. Observed cardboard lining the floor in front of the three compartment sink. Cardboard is not approved to line surfaces as it is absorbent and not cleanable. Cardboard was discarded.

Non-food contact surfaces of equipment are unclean. Observed build-up and food debris on dry storage shelves. Observed build-up on dish storage shelves above the three compartment sink. Observed build-up/food debris inside the 2-door freezer. Clean non-food contact surfaces so they are free of build-up and food debris.

Repeat Physical facilities not maintained in good repair. Observed broken floor tiles throughout the facility: Under grill, by 3 door cooler, by kitchen entrance, around grease trap. Repair/replace floor tiles so the area is smooth and easily cleanable
