[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Food not properly protected from contamination by separation, packaging, and segregation. Observed raw shell eggs stored in with produce in the same container on the prep top cooler. Raw shell eggs are to be stored below (on the bottom shelf) all other raw meats, produce, or RTE foods. Corrective Action: All of the produce stored in the container with the eggs was discarded at the time of the inspection. Proper storage order was gone over wit the staff. Eggs are to be stored on the bottom shelf of the prep top cooler from now on.

3717-1-03.3(H) / Reheating for hot holding.
Critical Repeat Improper reheating of food for hot holding. Observed the facility reheating foods in bulk. The facility is not licensed for cooling and reheating in bulk. All bulk reheating of cooled foods is to cease. Facility is licensed to make foods and sell the foods on that same day. Discussed this with staff and talked about ideas to aid in their sales, how to cool correctly was also gone over in detail. Corrective Action: All bulk reheating will be eliminated as of today.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed RTE TCS foods that were not date marked. All RTE TCS foods must be date marked to reflect the seven days in which they must be sold, discarded, or consumed. This includes all cooked in house items (meat loaf, etc), cut leafy greens and tomatoes, and all ready to eat commercially prepackaged foods that you open (spaghetti sauce, lunch meats, etc). Corrective Action: Some of the foods were discarded at the time of the inspection, and others were properly date marked at the time of the inspection. Date marking was gone over with the staff and must be correctly implemented.

3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Employee eating, drinking, or using tobacco in non-designated area. Observed employees eating/drinking while preparing foods. Employees are not permitted to eat or drink during food preparation or in food preparation areas. All beverages were removed.

3717-1-04.1(Y) / Temperature measuring devices.
Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Observed that not all of the coolers in the facility had thermometers. Provide thermometers in all coolers so that the ambient air temperature can be monitored.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed a gap under the front exterior door and several holes in the back exterior door. Provide a door sweep on the bottom of the front door. Patch/cover all holes in the back door that the new exit bar does not cover (on order, installed soon). All exterior openings must seal tight to prevent the entrance of pests.

3717-1-04.8(E)(1) / Equipment, utensils, linens - storage.
Observed single service items stored below chemicals and toxic items. Rearrange the shelves so that single service items are not below and chemicals, toxic items, or beauty products.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Repeat Re-use of single-service or single-use articles. Observed the facility re-using single service tin pans. Single service items cannot be washed and/or re-used. Single service items are to be used and discarded after one use. All washed tin pans were discarded at the time of the inspection.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed peg board on the wall behind the coolers. Remove the peg board as it is not cleanable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed split/torn door gaskets on the doors of the walk-in cooler. Replace all of the damaged door gaskets.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observe the dry storage shelves in the back stock room made of bare wood. Paint the shelves or seal them. Bare wood is not permitted in the facility as it is absorbent and not smooth or easily cleanable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed the 1 door upright freezer in the kitchen not functioning. Repair the freezer so that it functions correctly.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed the facility reusing cardboard boxes for storing food and equipment. Once empty of original contents cardboard boxes are not to be reused. Provide washable containers for the proper storage of food and or equipment.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed bare wood shelves and cardboard in use ion the walk-in cooler bare wood and cardboard are not permitted as they are absorbent and not smooth or easily cleanable. Remove the cardboard and seal/paint the bare wood.

3717-1-05.4(H) / Toilet room receptacle - covered.
No covered receptacle in rest-room. Provide a covered trash can in the rest-room.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed broken/damaged/missing floor tiles. Replace all broken/damaged/missing floor tiles.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed that he walk-in cooler floor needs to be cleaned especially under the shelves. All shelves must be 6" up so that proper cleaning can be done.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed damaged walls throughout the facility including in back stock. Repair, sand, and paint all of the damaged wall areas. Walls must be maintained in good repair and be smooth and easily cleanable.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. Observed the chargrill no longer in use or connected to the gas supply. Remove the chargrill from the facility as it is no longer necessary.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Repeat Light intensity less than ten foot candles in required areas. Observed .7 foot candles of light in the walk-in cooler. Walk-in coolers are required to have 10 foot candles of light or more to allow for proper cleaning food quality inspection. Provide 10 foot candles of light in the walk-in cooler.

Met with Ash, Beatrice, and Jennifer during the inspection. All handouts were left at the time of the inspection.

Facility has been doing a lot of work and is much improved, and plans to do a lot more in the near future.

Discussed that all changes to the facility, the equipment, or the foods/beverages offered must be submitted in writing to the health department and pre-approved prior to the changes being made in the facility. Contact your inspector with questions.
