[?] A summary of the violations found during the inspection are listed below.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
No towels or drying device at the handwashing sink(s). Provide paper towels at the handsink in the rest-room at all times.

3717-1-06.1(N) / Exterior walls and roofs - protective barrier.
Repeat Perimeter walls and/or roof do not provide adequate protection from weather, insects, rodents, animals, etc. Observed the buildings roof and soffit in terrible repair. Owner states that an estimate date for the work is already scheduled, and all the repairs will be done soon.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed broken cooler door handles. Manager states that the cooler doors are scheduled to be replaced.

Repeat Equipment components are not intact, tight or properly adjusted. Observed split and torn gaskets on the beverage cooler doors and on the Stewart Sandwich cooler door. Replace all of the split/torn cooler gaskets.

Equipment components are not intact, tight or properly adjusted. Observed the inner pane of glass on one of the walk-in cooler doors broken/shattered. Replace the glass in the door. Manager states that the glass is on order.

3717-1-04.4(D) / Warewashing equipment - cleaning frequency.
Warewashing equipment and/or components not cleaned at required frequency. Observed the 3 compartment sink in need of a deep cleaning. Clean the sink and then maintain it clean.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the faucet on the rest-room handsink spraying. Manager is repairing it. Observed the sprayer no longer connected on the faucet of the 3 compartment sink. Repair and/or replace the sprayer. Maintain all plumbing in the facility in good repair and functioning correctly.

3717-1-05.1(I) / Service sink - number.
Repeat Service sink or curbed cleaning facility not provided and/or conveniently located. Facility removed the mop sink. Provide a functioning mop sink in the facility. All plumbing must be done by a licensed plumber to the Ohio Plumbing Code with all necessary permits pulled and inspections performed.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Improperly cleaned storage area for refuse, recyclables, or returnables. Observed the facility with a grease dumpster that is no longer in use or required. Remove the grease dumpster.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Remove all unnecessary items from the facility. This includes personal items and equipment that is no longer necessary to operating the facility.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the air vents and air filters in the ceiling of the facility in need of cleaning and replacing. Clean the air vents and replace the filters.

901:3-4-16(C) / Level one Certification in Food Protection
Repeat RFE does not comply with Level One Certification requirements. Facility must have someone on site during all hours of operation that is food safety certified at the Person In Charge Ohio level. Provide copies of certificates on file in the facility to verify compliance.
