[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Repeat Insufficient air gap between the flood rim and the water supply inlet. Observed no air gap present at the prep sink. Install an air gap at the prep sink for backflow prevention.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection. PIC had Manager training, but it was not the ODH Manager certification required by Ohio Food Code. PIC was provided a link to obtain reciprocity for her education to get the ODH certification.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. The back door was open and a screen door was closed there as the barrier. Observed gaps around the exterior of the screen door that could allow insects, rodents, and/or other pests to gain entry to the facility. Install weather stripping and/or sweeps to eliminate the gaps around all exterior doors.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Repeat Corrected During Inspection Food not protected from contamination. Observed food, spring rolls, cooked breaded chicken, wantons, etc... uncovered in the walk in cooler. Unless food is actively cooling, it shall be covered when placed in the walk in cooler. PIC had all food items in the walk in cooler covered.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. Observed excessive clutter in the facility. Remove all unnecessary items from the facility.

3701-21-25(K) / Person in charge certification in food protection
FSO did not have a person in charge per shift with the person in charge certification in food protection.
