3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed a bucket of raw fish stacked on a sauce bucket. PIC said the fish was for personal use, and placed on the floor where it could not contaminate other food items Observed raw chicken above raw beef. Employee moved the beef and chicken so the chicken could not contaminate the beef.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Chlorine sanitizing solution at incorrect temperature and/or concentration. Observe low/no chlorine in the sanitizing bucket. PIC added chlorine so the sanitizing bucket tested between 50 and 100 ppm.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed multiple items in the facility not having date marks. Employee date marked all items in question. Items without date marking may be subject to discard in future inspections.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Repeat Corrected During Inspection Insufficient air gap between the flood rim and the water supply inlet. Observed no air gap at the prep sink. Employees placed a ring around the drain, removing the air gap. Employee removed the ring.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection. Observed no employee with ODH manager training in food protection.

3717-1-06.2(B) / Handwashing cleanser - availability.
Corrected During Inspection No soap at handwashing sink(s). Observed no soap at the restroom handwashing sink.

3717-1-03.4(C) / Thawing - temperature and time control.
Repeat Corrected During Inspection TCS foods not properly thawed. Observed shrimp thawing in a bucket of standing water. If food is thawing in water, the water shall remain running allowing it to thaw properly without temperature abusing the product.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Corrected During Inspection Food containers not properly labeled. Observed not identifying name on bulk food containers in the back of the facility. PIC wrote the name of the foods stored in each container.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Repeat Corrected During Inspection Food not protected from contamination. Observed multiple uncovered food items in the walk in cooler. Unless actively cooling, food items in the walk in cooler shall be covered.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the three compartment sink faucets leaking.
