[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection. Observed no evidence of an employee having ODH manager training in food protection.

3717-1-03.4(C) / Thawing - temperature and time control.
Repeat TCS foods not properly thawed. Observed a pan of chicken thawing at room temperature. Food is to be thawed under refrigeration, under cool running water, as part of the cooking process, or in a microwave if cooked immediately after thawing. PIC placed the chicken in the walk in cooler.

3717-1-06.1(M) / Outer openings - protected.
Repeat Corrected During Inspection Outer opening not protected. Observed gaps around the screen door, through which pests could enter the premises. Seal the gaps around the screen door, or keep the exterior door closed. PIC closed the exterior door.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Repeat Corrected During Inspection Food not protected from contamination. Observed uncovered food in the walk in cooler. Employee entered the cooler and began covering containers of food during the inspection. Ensure food in the walk in cooler is covered unless it is actively cooling.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Observed a knife stored between the prep table and the make table. PIC washed, rinsed, and sanitized the knife. Cease storing the knife between the tables.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed the racks in the walk in cooler being rusty. The rust makes the racks no longer smooth and easily cleanable. Remove the rust and reseal the racks, or replace them.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed food debris on the racks in the walk in cooler. Clean the racks and maintain them clean.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed excessive items in the facility that are not needed or allowed to be in the facility, such as multiple home style blenders. Remove unnecessary items and clutter from the facility.

3701-21-25(K) / Person in charge certification in food protection
Repeat FSO did not have a person in charge per shift with the person in charge certification in food protection. Observed no evidence of an employee present with person in charge training in food protection.
