3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed old dried meat on the slicer. Corrective Action: Completely disassemble the slicer after use and then properly wash, rinse, and sanitize the parts and the base. Maintain the slicer clean to the sight and touch.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the main food prep table in the kitchen area with multiple holes in the surface. Holes were caked with food/grease debris. Corrective Action: Replace the prep table as it is no longer in good condition and can no longer be maintained clean to the sight and touch.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed the breading flour sieve and the baskets in the flour dredge area with a build-up on them. Corrective Action: Thoroughly clean all equipment and containers that are used for breading once breading session is completed. Kitchen was not open or operating at the time of this inspection. Items were not cleaned after breading was completed.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the underside of the coffee brewer basket area not maintained clean. Corrective Action: Properly clean the coffee brewer and all parts at least once every 24 hours. Maintain the coffee pot clean to the sight and touch.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Observed the ambient air temperature of the walk-in cooler to be 51 degrees. Internal temperatures of items stored in the cooler were as follows: juice=45 degrees, sour cream=46.5 degrees, and milk 46 degrees. TCS foods must be maintained at an internal temperature of 41 degrees or less to prevent the growth of pathogens. Corrective Action: All of the TCS foods in the walk-in cooler were discarded at the time of the inspection and a service company was called to repair the cooler.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed gyro meat, chopped lettuce, and multiple loaves of open lunch meat without any date markings. All TCS RTE foods are required to be date marked to reflect the 7 days in which they must be sold, discarded, or consumed. Corrective Action: Facility stated that they would mark these items right away. In the future all items not properly date marked will be discarded at the time of the inspection.

3717-1-06.4(K) / Controlling pests.
Critical Facility is packed with storage in the entire back of the store, preventing proper cleaning and creating a possible pest harborage. One entire rest-room is completely full of items and the back hallway is almost not passable due to the stock. Corrective Action: Facility is to reduce their stock and store items up off the floor.

901:3-4-15(B) / Embargoing of food due to adulteration, misbranding, or expired baby formula or baby foods
Critical Food in an RFE must be embargoed due to adulteration, misbranding, or baby food or formula is expired. Observed outdated baby food being offered for sale, Suero Electrolyte Solution. Corrective Action: Facility discarded all of the out of date baby foods during the inspection.

Repeat Working food containers not properly labeled. Observed bulk food containers and squirt bottles in the kitchen area not marked with contents. Label all containers with their contents.

Repeat A food packaged in the facility not labeled. Observed the facility offering cups of ice for sale that were not labeled in any way. Label all of the cups of ice with the store name, store address, and net weight.

3717-1-06.1(M) / Outer openings - protected.
Repeat Corrected During Inspection Outer opening not protected. At the time of the inspection the front door was propped open. All exterior doors must be kept closed to prevent the entrance of pests. Door was closed during the inspection.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Improper storage of food items. Observed food being stored on the floor throughout the facility. All food must be stored at least 6" up off the floor to prevent contamination.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service or single-use articles. Observed the facility using single service cups for scoops in bulk ingredients. Provide washable scoops with handles for bulk food items. Single service items are permitted to be used once.

Single-service and single-use articles not protected from contamination. Observed single service items stored on the floor throughout the facility. Store all single service items up off the floor to protect them from contamination.

Equipment components are not intact, tight or properly adjusted. Observed the walk-in cooler not maintaining temperature. Have the walk-in cooler repaired so that it maintains temperature.

This issue is leaving large gaps that are not cleanable. Reattach the faucet to the sink so that it is correctly installed and the gaps are eliminated.

Equipment components are not intact, tight or properly adjusted. Observed the handle on the 2 door freezer broken. Repair or replace the handle.

Repeat Warewashing equipment and/or components not cleaned at required frequency. Observed the 3 compartment sink in a filthy state, large build-up of grease and black debris on the interior of the bays. Thoroughly clean and sanitize the 3 compartment sink and then maintain it clean.

Non-food contact surfaces of equipment are unclean. Observed a build-up of food debris and grease on the 3 compartment sink sprayer. Thoroughly clean the sprayer and then maintain it clean.

Non-food contact surface(s) not cleaned at the required frequency. Observed the exterior of the equipment in the kitchen area in need of cleaning. Clean the equipment and then maintain it clean.

Plumbing system not properly maintained or repaired. Observed that the sprayer on the 3 compartment sink does not turn off with handle on the sprayer itself. Have the sprayer repaired so that it functions correctly.

Repeat No covered receptacle in rest-room. Provide a covered trash can in the rest-room.

Repeat Plumbing fixtures unclean. Observed the mop sink and the area around it filthy. Clean the mop sink and the area around it.

Repeat Physical facilities not maintained in good repair. Observed water damaged ceiling tiles throughout the facility. Replace all water damaged ceiling tiles. Repair any and all roof leaks.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. Observed an old cheese pump and multiple fryer baskets sitting with an automotive battery on a shelf in the back stock hallway. These items have been sitting there for years. Remove all unnecessary items from the facility.
