Critical Food not properly protected from contamination by separation, packaging, and segregation. Observed foods in the walk in freezer are stored uncovered Cover food once it has cooled properly to prevent contamination.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed several bulk containers of salsa in the walk in cooler with very old date stickers. These containers have not been date marked for the newest batch of salsa. Stickers must be removed and containers properly washed, rinsed, and sanitized before using for another batch. The new batch of food must be properly date marked.

Critical Presence of live insects, rodents, and other pests. Observed small gnats or files in the kitchen, basement, and bar. Contact a licensed pest control operator for treatment.

Repeat Facility does not have an employee with manager certification in food protection.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Food not protected from contamination. Observed a sour cream squeeze bottle stored directly in lettuce in the prep top cooler. Do not store containers of food directly in foods to prevent contamination.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed torn door gaskets on the prep top cooler and the walk in cooler.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the floor in the dry stock room in the basement needs cleaned.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. Observed unused items and equipment in this facility. Remove all unsused items and equipment. This facility cannot be used as a storage unit.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed sections of floors and walls missing in the basement. Observed ceiling tiles missing in the basement. Observed some cove base tiles missing in the kitchen. Observed the cove base is damaged in the walk in cooler by the freezer door. Observed condensate still dripping from the seams of the walk in cooler ceiling. Repair all physical facilities.
