[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. The glass door cooler at the expo area is holding salsa at 52F and milk at 52F at time of inspection. PIC stated that they are aware that the cooler isn't working properly and have asked the beverage company for a new cooler. Discard out of temperature milk and salsa. TCS foods must be held cold at 41F or lower at all times. This cooler can no longer be used to store TCS foods such as milk and salsa.

Repeat Facility does not have an employee with manager certification in food protection.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed gaps under the exterior door to the patio and the exterior door in the basement where pests could possibly enter. Provide door sweeps or seals to close any gaps around exterior doors.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Improper storage of food items. Observed foods stored under dripping condensate in the walk in cooler. No food may be stored underneath the seam or evaporator unit that are dripping.

3717-1-03.2(R) / Food storage - prohibited areas.
Repeat Food stored in prohibited area. Observed facility is storing drink mixers in large quantities for other Cinco de Mayo locations in the basement in an area that is also being used for woodworking. No food may be stored in this room in the basement as it is being used for construction purposes. All food must be stored in designated food storage areas in the facility. If this facility will be storing food or beverages for any other locations, a warehousing registration is required from the Ohio Department of Agriculture.

Repeat In-use utensils improperly stored. Observed the ice scoop stored directly on top of the ice machine in the basement. Provide an ice scoop or holder that protects the scoop from contamination between uses.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Observed torn door gaskets on the walk in cooler. Replace any torn door gaskets so that doors properly seal and are easy to clean.

Repeat Equipment components are not intact, tight or properly adjusted. Observed the prep top cooler has torn door gaskets at time of inspection. Replace torn door gaskets to provide a good seal and so that the doors are intact, smooth, and easily cleanable.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed crates used as dunnage in the walk in cooler are dirty. Maintain all non-food contact surfaces clean at all times.

3717-1-05.3(I) / Other liquid wastes and rainwater.
Improper disposal of condensation and other nonsewage liquids. Observed condensate is dripping from the fan unit inside the walk in cooler and also from the cooler ceiling seams. Repair the cooler so that there is no condensate dripping into the cooler. If this cannot be repaired, it must be replaced.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed old restaurant equipment kept in the basement where there used to be a functioning kitchen. This equipment is no longer used and must be removed from the premises. PIC stated that the landlord owns the equipment. The premises should be free of items that are unnecessary to the operation or maintenance of the food service operation such as equipment that is nonfunctional or no longer used.

Observed the bar floor needs cleaned behind and beneath equipment and fixtures.

Observed ice build up on the fan unit in the walk in freezer. Clean ice build up and repair the freezer so that ice does not accumulate.

Observed water damaged ceiling tiles in the basement. Replace water damaged ceiling tiles and repair any leaks that may remain. PIC stated that there are currently no leaks.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Light intensity less than ten foot candles in required areas. There is no light in the old walk in cooler in the basement that is now used for storage. At least ten foot candles (one hundred eight lux) of light at a distance of thiry inches above the floor is required in walk in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.

Observed floors, walls, and cove base molding have been replaced in the basement where there was previous water damage
*The walk in cooler is dripping from the fan unit and also from the ceiling seams. This is a repeat violation. This must be permanently repaired or plans in place for repair within 30 days. If this cannot be repaired, it will need to be replaced.
