Observed Certificate of Occupancy from Lucas County Building Inspection

*The dish machine was run four times and did not reach at least 160F at the dish at the final rinse. Dish machine reached 155F on the maximum registering thermometer.
*Provide a maximum registering thermometer or thermal tape for verifying dish machine temperature
*The reach in upright cooler was brought in from the Monroe Street, Toledo location. This unit is holding proper temperature, however, the unit needs to be returned to the Toledo store. That store was approved with this cooler in place and it is required to be there. This cooler will not be accepted at the Airport Highway store for licensure, per Jodi Vaughan, Food Safety Supervisor at the Toledo-Lucas County Health Department.

Final pre-license inspection can be scheduled once the original refrigerator that was at this location is properly repaired or, if unable to be repaired, a new refrigerator is received and holding proper temperature and the dish machine is reaching proper final rinse temperature of at least 180F or 160F at the dish. If a new refrigerator will be used, provide make and model number if it is different from the unit that was approved during plan review.
