| Kristin was available for the inspection. The mobile was not in operation at the time of the inspection. 1. Discussed sanitizer and having it set up before operation. 2. Discussed wiping down and cleaning all surfaces before and after operation. 3. The mobile may operate in June of this year or before. 4. The license was not given to the operator because the operator over paid, per Kayla R. The license was not processed before the inspection. Discussed with the operators to use this report as an okay to operate. THE MOBILE IS OKAY TO OPERATE! 5. Discussed that the mobile must have permits to work in the city of Sylvania, Oregon, and etc. 6. Gloves, hairnets, and thermometers were available. 7. Quat Sanitizer tabs are being used for sanitizer. The tabs should not be expired and neither your test strips. |
