3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the chicken battering/flour totes left out a room temperature with a build-up all over them from battering raw chicken. All food contact containers must be maintained clean to the sight and touch. Corrective Action: These containers and the sifter must be cleaned after battering in the morning. The flour from the morning batch battering must be discarded. Properly wash, rinse, and sanitize the totes and the sifter as well as discard the flour.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the clean dish rack and the baskets on the rack that hold the cleaned utensils to have a build-up of duet and dirt on them. Corrective Action: Clean the rack, the baskets, and the utensils. Maintain the clean utensil storage area clean to the sight and touch.

Critical Equipment food-contact surfaces or utensils are unclean. Observed the cutting board on the prep top cooler and the area under the cutting board not clean to the sight and touch. Corrective Action: Thoroughly clean the prep top cooler cutting board and the area under it. If the cutting board cannot be cleaned due to wear and scoring it will have to be replaced.

Repeat A food packaged in the facility not labeled. Observed the facility offering cups of ice for sale without any label. Cups of ice must be labeled with the store name and address, and the net weight of the ice in the cup. Provide the required labels on the cups of ice or cease offering them for sale.

Working food containers not properly labeled. Label all squirt bottles, shakers, and bulk containers with contents.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed the front door of the facility open to the outside at time of inspection. Do not prop open or leave open exterior doors. Observed the back exterior door opening into the ally with a gap on the bottom. Provide a door sweep on the bottom of the door to prevent pests from entering.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Improper storage of food items. Observed foods being stored on the floor throughout the facility. All foods must be stored at least 6" up off the floor at all times to prevent contamination.

Repeat Observed single service items stored on the floor throughout the facility. Store all single service items at least 6" up off the floor throughout to prevent contamination.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed the faucet on the food prep sink not correctly installed/attached to the sink. Correctly install the faucet on the food prep sink.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment not approved by a recognized testing agency. Observed the facility using homestyle equipment and adding it to the facility without approval. Facility added a homestyle chest freezer and a toaster. Homestyle equipment is not allowed. Facility is to replace the freezer and the toaster with commercial NSF approved equipment.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed the food prep table in the facility with multiple holes through the top of the table. The holes have a build-up of debris all around them and thus the table is no longer smooth and easily cleanable and must be replaced.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
Repeat No sanitizer test kit available. Provide a sanitizer test kit at all times in the facility for measuring the strength of sanitizer in the 3 compartment sink and in wipe towel buckets.

3717-1-04.4(D) / Warewashing equipment - cleaning frequency.
Warewashing equipment and/or components not cleaned at required frequency. Observed that the 3 compartment sink in a filthy state. Clean the 3 compartment sink and then maintain it clean. The 3 compartment sink is required to be cleaned and sanitized at a minimum at least daily or more often if it becomes contaminated.

Repeat Non-food contact surfaces of equipment are unclean. Observed the equipment n the facility in need of cleaning on the interior and exterior. Clean all of the equipment inside and out. The interior of the oven has dried spilled food inside, the freezer has dried spilled food inside, fryers need a deep exterior cleaning, front of prep top cooler needs cleaned. Clean all of the equipment and then maintain it clean.

Repeat Non-food contact surfaces of equipment are unclean. Observed the shelves on the isle across from the walk-in cooler to be dusty and dirty. Clean the isle shelves and then maintain them clean.

3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Plumbing fixtures unclean. Observed the mop sink and the area around the mop sink to be filthy. Thoroughly clean the mop sink and the surrounding area and then maintain it clean.

Plumbing fixtures unclean. It was observed that the one rest-room full of stock of clothing to the point that it cannot be entered. The rest-room has a terrible odor and the somewhat visible handsink is filthy. Clean the rest-room. Provide shelving for the clothing so that the rest-room can be properly cleaned and entered to clean it.

No covered receptacle in women's rest-room. Provide a covered trash can in the functioning rest-room.

Repeat Facility not maintained clean. Observed the floor throughout the facility in need of cleaning. Clean the floor and then maintain it clean.
