[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(1) / PIC: Assignment of Responsibility
Critical No person in charge present in food facility during inspection. Observed that only one employee was on-site and they did not have any form of food safety training or knowledge. Corrective Action: Provide at least one employee who is in the operation a majority of the time who is certified in food safety at the manager's level. Employees without any food safety training or knowledge cannot be in the facility all alone while it is operating.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. Observed that Skyy who was on duty alone at the time of the inspection did not have an employee health reporting form on-file. Corrective Action: All current employees must be trained in health reporting and the facility must provide proof in a verifiable manner that they were trained. Provide a signed copy of the Employee Health Reporting Form for all current employees.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Chlorine sanitizing solution at incorrect temperature and/or concentration. Observed that the chlorine bleach in the spray bottle was way too strong. Corrective Action: Use your test strips to ensure that all chlorine bleach solutions are at 50-100 ppm. Dispose of the contents of the spray bottle and re-mix to the proper concentration.

3717-1-02.4(A)(2) / PIC - Manager Certification
Facility does not have an employee with manager certification in food protection. Observed that the facility could not produce a certificate for a current employee who is on-site a majority of the time the facility is operating that is food safety certified at the manager's level. Facility must have at least one employee who is certified at the manager's level and is on site a majority of the time. This person cannot be counted twice, thus they cannot be the certified manager in more than one location. Provide proof of hours worked at the facility and proof of certification.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
No written procedures for responding to vomiting or diarrheal events. Observed that the facility did not have a written procedure for responding to a vomit/diarrheal event. Handout is attached to the inspection.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
No towels or drying device at the handwashing sink(s). Observed that there were no paper towels at the handwashing sinks in the kitchen, dining area, or in the first rest-room. Provide paper towels at all of the handwashing sinks in the facility at all times. Paper towels are to be available at all times to allow for the proper handwashing.

3717-1-03 / Food - safe, unadulterated, and honestly presented
Food was unsafe, adulterated, or not honestly presented. Observed the facility with outdated baby food. Baby food was date marked 1-31-2024. Baby food was discarded at the time of the inspection. Monitor dates on the baby food and discard all baby foods prior to expiration.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed a large rusted out section on the bottom of the back exterior door. Light from the outside was visible. Replace the door or seal/cover the rusted out section. All exterior openings must seal tight to prevent the entrance of pests.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed the facility re-using cardboard boxes for storing foods in on the dry storage shelves. Once empty of original contents the box is to be discarded. Provide washable containers for the storage of foods.

3717-1-04.4(D) / Warewashing equipment - cleaning frequency.
Warewashing equipment and/or components not cleaned at required frequency. Observed that the 3 compartment sink was in a dirty condition during the inspection. Appeared that the water and debris was in the sink from the previous day. The children had not had breakfast yet and the sink had standing old dirty water in the bay(s). Thoroughly clean and sanitize the 3 compartment sink and then maintain it clean.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the drain line on the 3 compartment sink leaking. Water was dripping out of the drain line and being collected in a pan on the floor, water was also dripping onto the floor. Have the drain line on the 3 compartment sink repaired so that it is free of leaks. Maintain all plumbing in good repair.

3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Plumbing fixtures unclean. Observed the handwashing sink in the infant room in need of cleaning. Clean the sink and then maintain it clean

3717-1-06.4(F) / Drying mops.
Mops dried improperly. Observed mops sitting in buckets. Hang all mops to dry properly. Organize the mop closet so that the mops can be hung when drying.

3701-21-27 / Embargo of Food
Adulterated or misbranded foods or expired infant formula or baby foods embargoed in an FSO. Observed out dated baby food in the facility, date was 1-31-2024. Baby food was voluntarily disposed of at the time of the inspection. Monitor all baby food and dispose of it prior to expiration.

Met with Skyy during the inspection. Facility had 2 children in attendace at the time of the inspection. Breakfast, lunch and snacks are served to the children. Facility is only open on Saturday and Sunday.

***Facility is operating without a license. License expired on March 1, 2024. Multiple notices have been sent to the location and the inspector has made contact with the owner. Facility cannot operate without the required FSO license. Owner needs to take care of this issue in person at the health department right away. If you continue to operate without a license, legal action will be taken.****
