[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed raw shell eggs stored above milk and onions in the walk in cooler. Observed containers of condiments stored in the edible ice bin at the bar.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. Observed ground beef dated 11/2 and queso dated 10/22.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.
3717-1-06.4(K) / Controlling pests.
Critical Observed the presence of flies in the kitchen. Observed the window that is open in the kitchen has no screen. Observed a fly strip in use by the dish machine.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions. To prevent possible contamination, fly strips are not permitted to be used in areas where there is food, food contact items, or clean equipment.

3717-1-03.4(C) / Thawing - temperature and time control.
Corrected During Inspection TCS foods were not properly thawed. Observed shrimp thawing in standing luke warm water in the food prep sink.
TCS food shall be thawed as required in this rule. TCS food shall be thawed: (1) Under refrigeration that maintains the food temperature at 41°F less; (2) completely submerged under running water at a water temperature of 70°F or below, with sufficient water velocity to agitate and float off loose particles in an overflow, and for a period of time that does not allow the food to exceed 41°F, or for a period of time that does not allow thawed portions of raw animal food requiring cooking to exceed 41°F; (3) As part of a cooking process if the food that is frozen is cooked to the proper temperature, or thawed in a microwave and immediately transferred to conventional cooking equipment with no interruption in the process; or (4) Using any procedure if a portion of frozen ready to eat food is thawed and prepared for immediate service in response to an individual order.
3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed tortillas stored in t-shirt bags on top of the steam well at time of inspection.
Provide commercial grade food containers or food-grade bags for storage of food items.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the ice machine lid is coming apart, exposing insulation in the lid. Observed the keg cooler at the bar is leaking liquid. Observed the walk in cooler door gasket is torn.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed an accumulation of debris/grease/food on the inner surfaces of the flat top grill where the facade has come off. Observed build up on the surfaces of the storage racks. Observed buildup on the surfaces of the can opener.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
Observed a leak in the plumbing underneath the dish machine.
A plumbing system shall be properly maintained. Properly repair the leak.
3717-1-06.4(A) / Repairing.
Observed floor tiles missing underneath the pop box system in the basement. Observed ceiling tiles that are damaged in the basement.
The physical facilities shall be maintained in good repair.
