Critical Equipment food-contact surfaces or utensils are unclean. Observed a dirty knives on the knife rack. Corrective Action: Equipment food-contact surfaces shall be cleaned to sight and touch.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Ready-to-eat, TCS food not properly discarded when required. Observed ham date marked 1/14 and 1/16. Corrective Action: Items were discarded.

Nonfood-contact surfaces not easily cleanable. Observed storage dish rack no longer smooth and easily cleanable. Observed storage dish rack flaking pieces of plastic.
