3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed need for owner to install and maintain this program. Shared handout: Conditional/Foodservice Employee Reporting Agreement. PIC states that they have not signed a training document like the ODH handout.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are dirty. Observed dirty ice maker (e.g. drip edge), at bar serivce one ice bin has the beverage gun hose making contact with ice as well as the mult-ibin of cut items-fruits/olives (SKYYVODKA) for drinks (underside of unit is sitting atop the ice in ice bin). Want to store the two 5-gallon ice pails inverted and off the floor. Observed ice maker's scoop resting on wet dirty surface on top of unit/wax paper type needs replacement-keep utensils in a 'sanitary' manner.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

Food employee(s) did not have their hair effectively restrained. Observed need for long hair to be restrained at bar service area.
To prevent contamination, food employees shall effectively restrain or cover hair to keep their hair from contacting exposed food, clean equipment, utensils or linens or unwrapped single-service or single-use articles.
3717-1-02.4(C)(16) / Person in charge: duties - ensure the facility has written procedures for vomiting and diarrheal events.
Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events..Observed the need for owner to install and maintain this program. Shared handout: ODH Clean Up of Vomit/Fecal Accidents.
Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.
3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. Observed need to replace ceiling tiles (above ice maker area and throughout other sensitive areas. Penetrations in walls and floors also need sealing.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.1(M) / Outer openings - protected.
Repeat Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Observed need to seal back door to patio area.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.

Discussed handwashing-no jewelry or items at wrists.

Measured 200 ppm quat. ammonia (make up with 65 F to 75 F water, measured 95 F and quat. ammonia will burn off in 20 to 30 minutes). Want to have good expiration date on test strips for sanitizer verifications.

Backroom has 3 compartment sink and dedicated handsink, but not in service. Owner has free BBQ/Grilled food in patio on Tuesday's.
