3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed food debris on the can opener blade. Clean the can opener immediately after using to prevent accidental cross contamination.

3717-1-02.4(A)(2) / PIC - Manager Certification
Facility does not have an employee with manager certification in food protection.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed. Observed the two door true freezer not working. PIC stated it was replaced with a one door freezer. Repair or remove the non-functional freezer.

3717-1-04.2(J) / Cleaning and Sanitizers - Availability
Cleaning agents and/or sanitizers not available in the FSO or RFE. Observed no sanitizer made up, or sanibucket/spray available at time of inspection. A sanitizer bucket/spray shall be made available prior to food prep or service.

Non-food contact surfaces of equipment are unclean. Observed the bottoms of the reach in cooler and freezer in need of being wiped out.
