3717-1-02.4(C)(4) / PIC: duties - ensure food employees are effectively washing their hands

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Repeat TCS foods not being cold held at the proper temperature. Observed cheese and steak pieces holding at 47 degrees in the prep top cooler by the handwashing sink. The steak and cheeses were discarded.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection. Observed no evidence of an employee dedicated to the facility with Manager training in food protection.

3717-1-06.2(B) / Handwashing cleanser - availability.
Repeat No soap at handwashing sink(s). Observed no soap at the front counter handwashing sink.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Repeat No towels or drying device at the handwashing sink(s). Observed no paper towels at the front handwashing sink.

3717-1-03.4(E) / Cooling methods - temperature and time control.
Improper method for cooling TCS foods. Observed the make table by the handwashing sink unable to maintain food below 41 degrees. Repair the make table so it can be trusted to maintain food below 41 degrees.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Improper storage of food items. Observed boxes of food stored on the floor in the walk in cooler. Store all food and/or food contact items at least six inches off of the floor to prevent accidental contamination from the premises, or while mopping.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed rust on the racks in the walk in cooler, making them no longer smooth and easily cleanable. Remove the rust and reseal the racks, or replace them.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed the front merchandiser cooler in need of being wiped out. Observed the drying racks being sticky, in need of being cleaned so they do not contaminate clean dishes stored on them. Observed the racks in the walk in cooler having food debris on them, thus in need of being cleaned.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed multiple light bulbs in the facility being burnt out and in need of being replaced. Observed the light fixture above the facility work desk not working and in need of replacing bulbs and/or repair. Observed the oven hood lights being out, and in need of repair/replacement. Observed the seal behind the three compartment sink in need of being scraped out and replaced due to a build up of a black, mold-like substance.

901:3-4-16(C) / Person in Charge Certification in Food Protection
Repeat RFE does not comply with Person in Charge Certification requirements. Observed no employee in the facility with Person in Charge training in food protection. The facility holds a risk class III food license, which requires someone on the premises at all times with a minimum of person in charge training in food protection. If this level of education cannot be maintained, the facility may need to consider operating under a lower risk class license.

A reinspection shall occur around noon on Wednesday, 04/24/2024, at which time all violations shall be corrected and/or reasonable timelines for correction provided.

Failure to do so may result in a summons to the Toledo Lucas County Health Department to explain the facility's lack of compliance, which is the first step towards revocation of the facility's food license.
