[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection. Facility must have a person on-site 51% of the time you are open and operating that is certified at the managers level in food safety. The remainder of the time a person certified in food safety at the Person In Charge Ohio level must be on-site. Provide certificates on-site to verify certifications. Handouts were left at the time of the inspection.

3717-1-03.5(C)(2) / Food labels - packaged food
Label on a food packaged in-house did not contain all required information. Observed cups of ice packaged in house not labeled at all. Cups of ice must be labeled with the facility name, physical address, and net weight. Provide labels on all cups of ice or stop selling them.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed foods being stored directly on the floor throughout the facility. All foods must be stored at least 6" up off the floor.

3717-1-04.8(E)(1) / Equipment, utensils, linens - storage.
Observed single service/single use items stored on the floor in the facility. Store all single service/single use items at least 6" up off the floor.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment not approved by a recognized testing agency. Observed the facility using multiple coolers and freezers that are not commercial or are not approved to hold TCS foods. All equipment in the facility must be NSF approved commercial equipment that's approved for the use the facility is using it for. Pepsi/Coke coolers are approved for canned non-TSC beverage storage. Emerson chest freezer is homestyle. When replacing equipment contact the health department so they can verify that the equipment meets the standards for what you are using it for. As equipment is replaced in the facility it must be replaced with approved equipment. Provide the health department with make and model numbers for all potential equipment changes in the future.

3717-1-04.4(B) / Cutting surfaces.
Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed the cutting board on the prep-top cooler stained and with surface issues. Replace the cutting board on the prep-top cooler as it can no longer be effectively. cleaned and sanitized.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed the fan guards and area around the fan guards in the walk-in cooler with a dust build-up. Clean and then maintain the fan guards and area around them clean.

Non-food contact surfaces of equipment are unclean. Observed a build-up of dust and debris on the shelves across from the beer walk-in cooler. Clean the shelves, then maintain them clean.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed water damaged ceiling tiles in the facility. Replace all water damaged ceiling tiles.

901:3-4-07.0 / Facility layout and equipment specifications for RFE
RFE constructed, altered, and/or equipment added without prior approval from the licensor. A homestyle chest freezer was added to the facility without prior approval and does not meet the requirements of being commercial.
