[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Food not properly protected from contamination by separation, packaging, and segregation. Use proper storage order in the meat grinder walk-in cooler to prevent cross contamination. Buns and hot-dogs were located under raw meats. Corrective Action: Owner was instructed to move the items.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Repeat No towels or drying device at the handwashing sink(s). Provide paper towels at all of the handsinks at all times. Provided while we were there. All of the sinks had them except for one in the meat display area.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected.Provide a door sweep on the exterior door in the storage room. Light from the outside was visible.

3717-1-06.1(K) / Insect control devices - design and installation.
Improper use or placement of insect control devices. Observed bug zapper in the meat room. Remove the bug zapper from the meat room as it does not have a way to contain the insects that are zapped.

3717-1-04.8(F) / Storage - prohibitions - single-use/single-service articles.
Observed single service articles and cases of olive oil being stored in the basement. Owner informed that single service items cannot be stored in the basement. PIC stated that Olive oil was for personal use. PIC stated that the items would be removed.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed broken and taped glass on the meat display case on the back sliding doors. PIC stated that new glass doors are on order.

Equipment components are not intact, tight or properly adjusted. Observed damaged gasket on the door of the prep top cooler. Replace the torn gasket on the cooler door.

Equipment components are not intact, tight or properly adjusted. Observed racks in the walk-in freezer that were bent and corroded. Replace the shelves in the walk-in freezer.

Non-food contact surfaces of equipment are unclean. Observed the prep table in the walk-in cooler that the grinder is sits on with a build-up. Thoroughly clean the prep table.

Non-food contact surfaces of equipment are unclean. Observed a buildup on the floor of the walk-in freezer. Clean the floor of the walk-in freezer.

Non-food contact surfaces of equipment are unclean. Observed the prep table in the three compartment sink room with a build-up on it. Thoroughly clean the prep table.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the wall coving no longer attached to the wall behind the fryers. Reattach the coving and thoroughly clean the floor and wall under and behind the fryer area.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed unnecessary items in the facility. Continue to remove all unnecessary items from the facility.

3717-1-06.1(F) / Wall and ceiling - coverings and coatings.
Unapproved materials covering the walls and/or ceiling. Observed cardboard taped to the wall at the entrance to the meat case area. Remove the tape and the cardboard replace it with mortar and then paint the area. Area must be smooth and easily cleanable.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed the ice machine is no longer functioning. Remove the ice machine and the water filter that use to supply water to it.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed a build-up in the flour storage room. Thoroughly clean the entire room and the back-up grinder that is located in it.

Met with Ishmail during the inspection.

Consulted: If the Minute Maid cooler is not removed shortly you'll have to paint the wooden peg board located inside of it. PIC stated that the cooler is being replaced.

Consulted: Notified the PIC that shelves cannot be lined with newspaper or cardboard. PIC corrected at the time of this inspection.

Consulted: Talked to PIC about sending one employee from the meat counter to a PIC course. PIC said he would select one of them to get certified.

Note: Facility has a PIC and a level II certified employee on staff.
