Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed black build up in the ice machine. Establish a more regular cleaning schedule for the ice machine, as this is a repeat issue for the facility.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
No written procedures for responding to vomiting or diarrheal events.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit available.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. Observed prep top cooler, and other food prep/storage equipment and utensils in the back kitchen area. Remove all unused equipment.

FSO license not displayed; improper display of mobile FSO information. Observed no food license prominently displayed in the facility.

Comments
When entering the paperwork portion of the inspection, PIC stated they did not have a food license, nor did they need one to maintain their liquor license as Shadow Valley is a private club. They stated the license was allowed to lapse last year due to covid, and they chose not to renew it due to no longer serving food and they did not want to replace the acoustic tile in the kitchen area. As all of the food is prepackaged and nothing is being prepared, the acoustic tile is no longer an issue.

A call was placed to Bryan Roger, liquor control, who stated Shadow Valley needed a food license to maintain the liquor license they hold.

Additional follow up was performed with the TLCHD regarding the status of their food license. Shadow Valley does indeed have a food license and never let it lapse at any time.
