Inspector was called out to a fire at the location by TFRD through Engage Toledo. TFRD was still on-site went the inspector arrived. TFRD stated that the fire was contained to the attic and believed to be an electrical fire. Ceiling and roof damage were done.

Toledo Edison, Dave, arrived on-site while the inspector was there and cut the main power supply to the facility.

Inspector instructed the facility that all TCS were to be disposed of including the following: lunch meats, cheeses, all dairy products, all ice cream, bacon, sour cream and dips, butter/margarine, all of the Stewart Sandwiches.

The following items must also be discarded due to potential contamination: bags of onions, and any other product that the packaging would not permit proper washing of the exterior or that would have been permeable to the water/smoke/or debris from the fire and resulting extinguishing of the fire. The fryer in the back room was full of oil and that also must be discarded. All open foods or foods subject to the entrance of water and or smoke in the back room must be discarded (bags of flour, open sauces, all open single service items tin plans/plastic wrap/baggies, and all open spices.

Facility is not licensed to cook foods on-site and they were instructed multiple times to remove all of the cooking equipment from the back room. All of the cooking equipment is to be removed from the facility including the grease dumpster outside. The entire back room area is to be cleared out of all equipment except for the sinks and the hood.

At the time of this inspection it was clear that the facility was cooking on-site. Facility is not licensed for on-site cooking. All on-site cooking is to cease and all of the equipment is to be removed.

Employees are not permitted to cook on-site. Employees are to pack a meal that would cover them for their shift.

Inspector notified Toledo Building Inspection of the fire.


Facility is not permitted to reopen or to operate until they have received permission/approved by Toledo Building Inspection and have been reinspected by the Toledo Lucas County Health Department and have been cleared to open and operate. All applicable permits must be pulled and finaled out through Toledo Building Inspection.

Due to the extent of the damage and the lack of electrical power facility is closed. The only people permitted to be inside are workers repairing/cleaning the facility.
