Facility not maintained clean. Observed some debris beneath the dry storage racks. Observed dust build up on the fan on the wall in the dish room.

Physical facilities not maintained in good repair. Observed some low grout underneath the dish machine where water and debris can accumulate. Repair grout so that floors are smooth and easily cleanable.
