[?] A summary of the violations found during the inspection are listed below.

3717-1-05.4(H) / Toilet room receptacle - covered.
Observed no covered receptacle in women's restroom.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulbs or heat lamp that were not properly shielded or coated where required.Observed missing light shield inside walk in cooler.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
3717-1-06.1(M) / Outer openings - protected.
Observed an outer opening, window, or door without proper protection against the entry of insects and rodents.Observed main entrance door to outside not sealed properly at time of inspection.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
3717-1-06.2(B) / Handwashing cleanser - availability.
Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s) in both ladies and men bathrooms.
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Observed no towels or hand drying device at the handwashing sink(s) in the men's bathroom during inspection.
To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.
3717-1-06.4(F) / Drying mops.
Observed mops being dried using an unapproved method.Observed mop being dried in a mop bucket in back storage room area at time of inspection.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.

Observed hand sinks in both ladies and men bathrooms.
Facility do not sell/serve hot food or coffee at this time.
Frozen temps met the Ohio code -1.5F to -3.5F
Refrigeration temps were good. ( milk and other TCS foods are held at 41F and below).
Facility do not sell raw shell eggs at this time
Note:Ensure to thoroughly clean build on the floor in back room area, replace missing fan guards inside walk in cooler, all boxes shall be stored six inches off the floor and ensure to clean dirt and debris underneath reach in coolers.
