[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(L) / Mechanical warewashing equipment - hot water sanitization temperatures
Critical Hot water for mechanical sanitization is below required temperature. Observed the mechanical warewashing machine unable to generate water at the proper temperature to sanitize dishes. PIC stated she would call and have the warewashing machine serviced so dishes could be properly sanitized.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed black and pink build up on the interior slide of the ice machine. Increase the cleaning frequency of the ice machine so build up does not occur.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Observed iTCS food tems in the prep top of the middle cold rail holding at 47 degrees. All TCS food items in the prep top were discarded. Cease using the middle cold rail until it is repaired. If TCS food items are placed in there, holding by time instead of temperature, as per the facility policy for other items by the prep top.

3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects, rodents, and other pests. Observed flying insects by the warewashing machine and ice machine. Hire an exterminator to eliminate the flying insects.

3717-1-03.4(E) / Cooling methods - temperature and time control.
Improper method for cooling TCS foods. Observed the prep top portion of the middle cooler unable to maintain food below 41 degrees. Service the cooler so it is able to maintain food below 41 degrees.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed a gap at the back door through which pests could gain entry to the facility. Install sweeps, sealants, and/or weather stripping to eliminate the gaps through which pests could gain entry to the facility.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Repeat Nonfood-contact surfaces not easily cleanable. Observed some racks in the walk in cooler being rusty, and thus no longer being smooth and easily cleanable. Remove the rust and reseal the racks, cover the racks, or replace them.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed some racks in the walk in cooler in need of being wiped down due to a build up of food debris on them. Observed the drying racks being tacky, in need of routine cleaning and being added to the cleaning rotation.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the middle bay of the three compartment sink leaking at the pipe underneath. Observed the digital display of the chemical dispenser blinking, in need of being serviced. Observed the water faucet handle leaking at the dish sprayer. Observed the north side server handwashing sink leaking hot water from the faucet. Employ a licensed plumber to address all of the facility's plumbing issues.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Repeat Improperly cleaned storage area for refuse, recyclables, or returnables. Observed wood and other refuse in the dumpster area. Clean the dumpster area and maintain it clean and free of refuse not in the dumpsters.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the lower wall, by the door in the dry goods area, in need of repair. Observed the metal baseboards, by the inside door of the walk in cooler, in need of being re-attached to the wall.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed cleaning needed under the ice machine. Observed the wall behind the warewashing area in need of cleaning. Observed foam/paper needing scraped off of the wall above the handwashing sink by the food prep area.

VI - P - TCS Food: Observed time/temperature controlled for safety foods being properly held using time as a public health control; the operation had a written procedure on file, foods were properly marked to indicate the time that the food was removed from temperature control, and food was discarded when required.
VII - P - Protection from Contamination: Foods are protected from physical and environmental contamination during storage, preparation, holding and display.
VI - P - TCS Food: Observed ready to eat TCS foods being properly date marked, and discarded when required.
III - P - Preventing Contamination by Hands: Observed food employee washing hands prior to donning single use gloves.
VIII - P - Consumer Advisory: Observed a consumer advisory on the menu for animal foods that are served raw, undercooked or not otherwise processed to eliminate pathogens.
I - P - Employee Health: The operation had an employee health policy on file.
X - P - Chemical: Toxic materials are properly identified and stored.
III - P - Preventing Contamination by Hands: Hand washing facilities are properly supplied.
