To obtain the Ohio Level 2 certificate, contact whomever you took the ServSafe exam with to see if they have this document. If they do not, then contact: Ohio Department of Health (614-466-3543) and ask about Reciprocity to obtain the Ohio Level 2 certificate. The Ohio Level 2 certificate must be available on-site for next inspection.
