[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(C)(4) / PIC: duties - ensure food employees are effectively washing their hands
Critical Person in charge did not ensure employees were effectively cleaning their hands as required.

3717-1-02.2 (C ) / Hands and arms: when to wash
Critical Food employee(s) did not wash hands when required. Observed employees touching trash bags and cans and go on to work with food or touch food contact surfaces without first washing their hands and changing gloves. Retrain employees on handwashing to avoid the spread of food borne illness.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Repeat TCS foods not being cold held at the proper temperature. Observed an over filled pan of sausage patties in the prep table, holding at 46 degrees. PIC discarded the patties until patties were found temping below 41 degrees. Cease filling pans above their designated fill lines.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Repeat Insufficient air gap between the flood rim and the water supply inlet. Observed no air gap present at one of the front ice bins. Install an air gap for backflow prevention.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
No towels or drying device at the handwashing sink(s). Observed no paper towels available at the front handwashing sink. Ensure paper towels are available at all kitchen handwashing sinks.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Improper storage of food items. Observed a box of food and two gallon jugs of milk on the floor in the walk in cooler. PIC moved all items off of the walk in cooler floor.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Repeat Food not protected from contamination. Observed no splash guard installed at the handwashing sink by dry goods. A box of coffee was stored below the sink and single service items were stored next to it, potentially being splashed during the washing and/or drying of hands. Install a splash guard to protect the single service items and food potentially stored in the area. If no splash guard is installed, move all food and/or potential food contact items so contamination from splashing cannot occur. Observed two uncovered bins of hot chocolate mix stored behind the front handwashing sink, between the sink and the paper towel dispenser. The mix would be contaminated, via dripping, whenever someone tried to wash their hands and get paper towels.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the drain pipe for one of the front ice bins leaking. Repair the leak.

3717-1-05.1(Q) / Scheduling inspection and service for a water system device.
Water treatment device or backflow preventer not inspected/serviced with documentation as required. Observed no date mark indicating the install or replacement date for the water filter.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the cleaning needed underneath the ice machine.
