3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Fixed equipment not properly sealed or spaced for cleaning. Observed the food prep sink is no longer attached and seal to the wall. Reattach the food prep sink to the wall and then seal behind it.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed a groove worn in the dough stretcher roller. Thus the roller is no longer smooth and easily cleanable. Replace the roller on the dough stretcher.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed a build-up on the floor of the walk-in cooler. Clean the walk-in cooler floor and then maintain it clean.
