Critical Repeat Food not properly protected from contamination by separation, packaging, and segregation. Observed uncovered tamales stored in the walk in freezer. Once foods have cooled they must be covered to protect them from contamination.

Repeat Facility does not have an employee with manager certification in food protection.

3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Handwashing sink water below 100°F in the women's restroom. Water was run for several minutes and only reached 80F. Adjust mixing valves to restore hot water of at least 100F.

3717-1-07.1(K) / Rodent bait stations.
Improper use of a rodent bait station. Observed d-con rodent bait used in the dry storage room. All rodent bait stations must be contained in a covered, tamper resistant bait station. contact a licensed pest control opeerator for treatment.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed a gap at the bottom of the door to the patio where pests could enter the facility. Provide a proper door seal to close this gap.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Repeat Food not protected from contamination. Observed pineapple double panned in the prep top cooler at the cookline area. To prevent contamination from the bottom of the container, do not stack open containers of food.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed food stored beneath a seam of the walk in cooler ceiling where condensation is dripping. Food may not be stored under this seam. Repair the dripping seam.

3717-1-03.2(R) / Food storage - prohibited areas.
Food stored in prohibited area. Observd boxes of tortillas stored in the basement dining area, which is presently being used as a workshop for cutting wood for dining room furniture.

3717-1-03.2(K) / In-use utensils - between-use storage.
Observed the ice scoop stored directly on top of the ice machine in the basement. Provide an ice scoop holder or a clean dry container for storage of the ice scoop.

3717-1-04.7(B) / Frequency of laundering - specifications.
Wiping cloths not properly laundered. Observed a soiled wiping cloth at the cookline area. Linens shall be laundered between operations if they become wet, sticky, or visibly soiled.

Repeat Equipment components are not intact, tight or properly adjusted. Observed torn door gaskets on the walk in cooler door and on the prep top cooler at the cookline area.

Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed the cutting boards at the prep top coolers are stained.

No covered receptacle in women's restroom.

Repeat Unnecessary or nonfunctional items and /or litter on premises in the basement. Remove all items unnecessary for the operation.

Observed the floor of the dry storage room in the basement needs cleaned.

Repeat Observed a missing cove base tile by the walk in cooler door in the kitchen.

Repeat Physical facilities not maintained in good repair. Observed missing flooring, walls, and ceiling in the basement from a past sewage back up that left water damage. All floors, walls, and ceilings must be intact, smooth, and easily cleanable.

Repeat Observed employee cell phone and hydrogen peroxide stored with and above food at the steam table area. All employee belongings must be stored in a designated area that is separate from or below all food and food contact items.
