3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Repeat Food not properly protected from contamination by separation, packaging, and segregation. Observed food in the reach in coolers and in the freezer that were not properly protected from contamination. Correction: To prevent contamination food shall be separated, packaged, and or segregated. PIC will cover all food items that are uncovered.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed that the door on the reach in cooler does not seal shut once it is closed.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed that the surfaces of the reach in coolers, and hot holding ovens have build up and debris on them. Also, the inside of the cold and hot holding equipment has debris on the bottom shelving that needs to be cleaned out.

3. Discussed and consulted about wiping all equipment down on the inside and the outside.
4. Pest control reports seen at today's visit.
