3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed food that was not properly protected from contamination by separation, packaging, and segregation. Observed employee drinks/food items on and above shelving of food items for customer use. Shall be stored separately to prevent contamination.
Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-07(B) / Working containers - common name.
Critical Observed working containers of chemicals that were not properly labeled with the name of the material. Observed spray bottle of Sanitizer without label at time of inspection.
Working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.
3717-1-07.2 / Poisonous or toxic materials: storage and display - separation
Critical Observed improper display or storage of poisonous or toxic materials. Observed sanitizer being stored on same shelf and above food items.
Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, single-service articles, or single use articles by separating the poisonous or toxic materials, and locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, single-service articles, or single-use articles.

3717-1-04.1(Y) / Temperature measuring devices.
Cold holding equipment did not contain a thermometer.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit.
3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Observed plumbing fixtures that were not being kept clean and maintained. Observed handsink with empty can in it at time of inspection.
Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean and maintained.
