3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed tomatoes delivered to the facility in a cardboard box. The ox did not indicate the produce was pre-washed and thus the tomatoes and box were not cleaned and sanitized. If the tomatoes were washed, then they should not be placed back into the dirty box, as that would recontaminate the tomatoes. Placed all pre-washed produce in food grade bags or containers.

3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Working containers of poisonous or toxic materials not properly labeled. Observed two spray bottles, with a blue chemical in them, without labels identifying the contents.
