[?] A summary of the violations found during the inspection are listed below.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are dirty. Observed build up inside the pop gun.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment is not approved by a recognized testing agency. Observed homestyle containers in the facility.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. Remove all homestyle containers like Gladware and replace with commercial grade containers.
3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. Observed acoustic ceiling tiles in the kitchen.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-09(B) / Facility layout and equipement specifications - acceptable equipment
Repeat Observed the use of equipment that has not had prior approval from the licensor. The Frigidaire dishwasher is still at the bar and a grill is in the back of the kitchen not under a hood.
Food equipment that is acceptable for use in a FSO or RFE shall be approved as specified under paragraph (KK) of rule 3717-1-04.1 of the Administrative Code. The dish washer and grill must be removed from the facility as they are not approved equipment by this agency. A referral to Toledo Fire Inspection will be made in regards to the grill.
