[?] A summary of the violations found during the inspection are listed below.

3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects, rodents, and other pests. Observed flying insects by the mop sink area. PIC stated she believes they are coming up from there. Hired an exterminator, licensed to work around food, to control the flying insect problem.

3717-1-02.4(A)(2) / PIC - Manager Certification
Facility does not have an employee with manager certification in food protection. PIC stated she would send a copy of her license, at which time this violation would be removed.

3717-1-04.5(B)(5) / Equipment food-contact surfaces and utensils - cleaning frequency.
Utensils and equipment contacting non-TCS foods not cleaned at required frequency. Observed black build-up inside the ice machine. Increase the cleaning frequency of the ice machine.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed water running out of the preptop cooler when the doors were opened. Repair/adjust the cooler so the interior does not generate water, as it can contaminate food, and/or present a slip/fall hazard to employees.

3717-1-04.2(H)(2) / Temperature measuring devices - mechanical warewashing.
Irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations. Observed no thermal labels or an irreversible registering thermometer in the facility to ensure the warewashing machine was sanitizing properly.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observed the entire facility in need of a deep cleaning.
