3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed pink build-up on the inside surfaces of the ice-machine. Reviewed with manager on proper cleaning and sanitizing frequency.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection assigned spefically to this store. At this time, only one person has a valid Level Two Manager Food Safety Certificate - Christopher Mowers, which covers all stores. This issue has been discussed on the previous SIX inpsections at this facility. One person per licensed facility must have a Level Two Food Safety Certificate.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed damaged flooring throughout the store, observed damaged surfaces in the chemical room - ceiling tiles stained, wall and flooring.

Masks - masks are required to be worn by all employees unless someone fits the exemption. If there is a medical exemption a written statement by a dr is required and maintain this documentation in personnel file.
Hair - hair must be restrained by a covering such as hairnet or hat.
