[?] A summary of the violations found during the inspection are listed below.

3717-1-02.2 (D) / Hands and arms: where to wash
Critical Food employee(s) not washing hands in approved handwashing sink or automatic handwashing facility. Observed that none of the handwashing sinks in the facility were functioning correctly and employees were washing their hands in the 3 compartment sink. Corrective Action: All of the handwashing sinks are to be fully functional and running without turning on the valves below the sinks. Employees are to properly wash their hands at the handwashing sinks and not in the 3 compartment sink.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed both of the meat grinders in the facility in a filthy state. Corrective Action: Thoroughly clean both of the meat grinders prior to use. Grinders are to be properly washed, rinsed, and sanitized. Grinders are to be stored in clean condition and protected from contamination between uses. Note the grinders were not in use at the time of the inspection.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed chicken wings stacked beyond capacity in the meat display case. Raw chicken wings were in contact with the interior surfaces and doors of the display case. Corrective Action: The contents of the meat display case was reduced and raw chicken was only contacting the cleaned and sanitized pans in the display case. Cease overstocking the display case.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed the facility using raw chicken cardboard boxes for breading and seasoning chicken in. All food contact surfaces must be constructed to be smooth and easily cleanable. Cardboard boxes cannot be properly washed, rinsed, and sanitized and thus cannot be used for food contact or food preparation. Corrective Action: All cardboard boxes were discarded at the time of the inspection. Facility started using their commercial NSF approved pans for breading and seasoning.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed the slicer with a build-up. Corrective Action: Slicer is to be broken down and cleaned after each use if it is not in constant use. If in constant use it is to be properly washed, rinsed, and sanitized at least once every 4 hours. Break done the slicer and thoroughly wash, rinse, and sanitize it.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed the ice machine with a build-up on the interior of the unit of a mold like substance. Corrective Action: Ice machine was turned off and the ice was discarded. The ice machine will be properly washed, rinsed, and sanitized prior to being turned back on and used. Facility is to maintain the ice machine clean to the sight and touch at all times.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed the facility not date marking their TCS RTE foods as is required by law when they hold them for more than 24 hours. Corrective Action: Date mark all TCS RTE foods held greater than 24 hours including: all lunch meat loafs, all cut lunch meats, cut tomatoes, and cut lettuce. All TCS RTE foods found without a date mark in the future will be discarded at the time of the inspection.

3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects, rodents, and other pests. Observed bird nesting materials on the ceiling of the back stock room. Corrective Action: Seal all outer openings to prevent the entrance of birds, and remove all of the nesting material and feces.

3717-1-03.5(A) / Standards of identity
Repeat Packaged food labels are not in compliance with standards of identity. Observed the facility offering Ritz and Saltine crackers by the sleeve. The sleeve of crackers do not meet the standards of identity to allow them to be sold. All foods offered for retail sale must be labeled with the name and address of the producer, the net weight of the contents, a list of ingredients, and all major allergens declared. Cease selling items that are not properly labeled for retail sale.

3717-1-03.5(C)(1) / Food labels - packaged food
Repeat A food packaged in the facility not labeled. Observed the facility packaging and offering for sale bagged ice and shucked corn on the cob. All foods packaged in house for retail sale must be properly labeled. Labels must include: Name and address of facility, net weight of product, a list of ingredients, and sub ingredients, all major food allergens declared.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed multiple outer openings in the facility that were not properly screened or sealed. All outer openings must be screened with 16 mesh per square inch or completely sealed so that they do not allow the entrance of pests. Screen all of the wall mounted exterior fans, the vented openings and /or windows, all exterior openings.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Repeat Improper storage of food items. Store all food in the facility at least 6 inches up off the floor to prevent contamination. This includes everywhere in the facility and in the walk-ins.

3717-1-03.2(G) / Washing fruits and vegetables.
Raw fruit and/or vegetables not washed or washed improperly. Observed employees washing vegetables in the 3 compartment sink and then placing the vegetables back into the dirty cardboard box they came out of. All produce must be washed in the food prep sink only and then placed into sanitized containers.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Repeat Re-use of single-service or single-use articles. Observed the ice scoop at the ice machine being stored in a candy container. Provide a multi-use commercial NSF approved container for storing the ice scoop in between uses. Single service items cannot be washed and reused, discard all candy containers once empty of original contents.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Repeat Re-use of single-service or single-use articles. Observed the facility re-using boxes that raw chicken came in originally. All boxes are to be immediately disposed of once empty of original contents. Boxes are not to be re-used for any purpose.

3717-1-04.4(B) / Cutting surfaces.
Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed worn and stained cutting boards in the facility. Replace all worn and stained cutting boards. Cutting boards must be NSF approved.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Repeat Nonfood-contact surfaces constructed of unapproved materials. Observed newspapers and cardboard lining the shelves throughout the facility. Shelving material must be non-absorbent, smooth, and easily cleanable. Newspapers and cardboard are not allowed on shelves as they cannot be properly cleaned. Remove all newspapers and cardboard fro the shelves throughout.

3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment not approved by a recognized testing agency. Observed the facility with and using multiple pieces of homestyle equipment. Homestyle equipment is not allowed in the facility. Provide commercial NSF approved equipment only. Remove the homestyle equipment including: toaster, air fryer, and rice cooker.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Nondurable equipment observed. Observed the facility using equipment for something other than the purpose that it was designed for. Remove all equipment that you are not using for it's intended purpose. Old 2 door freezer that no longer functions was being used for dry storage. Remove the nonfunctional freezer from the facility. Observed the old walk-in being used for dry storage, remove the walk-in from the facility.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit available. Provide the sanitizer test kit at all times for measuring the strength of the sanitizer.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed the interior and exterior of the following pieces of equipment in need of cleaning. Clean the equipment and then maintain it clean at all times. Clean the 2 door Migali freezer and the Hatco food warmer inside and out.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed the interior of the walk-in cooler in need of a deep cleaning. Clean the interior of the walk-in cooler and then maintain it clean.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed that the sump pump in the basement was turned off and the basement was flooding. Sump pump must function turn it on or repair/replace it. Thoroughly clean the flood up in the basement.

Repeat Plumbing system not properly maintained or repaired. Observed that none of the handwashing sinks were functioning correctly in the facility due to leaks and maintenance issues. All of the handwashing sinks are to function correctly at all times and be maintained in good repair. Repair or replace all of the handwashing sinks in the facility.

Plumbing system not properly maintained or repaired. Observed the drain line on the ice machine running up hill. Replace the drain line on the ice machine so that it has the correct fall per the Ohio Plumbing Code.

3717-1-05.4(G) / Storage areas, rooms, and receptacles - capacity and availability.
Adequate area and/or receptacles for refuse, recyclables, or returnables not provided. Observed the facility reusing cardboard boxes for trash cans throughout. Provide actual trash cans that are non-absorbent and smooth and easily cleanable for disposing of trash within the facility. Stop using boxes for trash.

3717-1-05.4(G) / Storage areas, rooms, and receptacles - capacity and availability.
Adequate area and/or receptacles for refuse, recyclables, or returnables not provided. Observed that the facility does not have any trash cans at the handwashing sinks for disposing of paper towels. Provide trash cans at all handwashing sinks. Cardboard boxes are not to be used as trash cans.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed the cement floor in the 3 compartment sink room no longer sealed. Deep clean the floor and then seal the cement so the floor is not absorbent.

Repeat Floor and wall junctures not properly coved or closed and/or floor drains not provided. Observed missing and falling off cove base throughout the facility. Replace all coving that is missing or falling off.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Mortar and paint the wall at the end of the meat display case. Walls must be smooth and easily cleanable.

Repeat Physical facilities not maintained in good repair. Repair or replace all of the damaged flooring throughout the facility. Flooring must be non-absorbent, maintained in good repair, and be smooth and easily cleanable.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed acoustic ceiling tiles in the fryer and 3 compartment sink rooms. All ceiling tiles in these two rooms must be vinyl faced, thus smooth, easily cleanable, and non-absorbent. Replace the acoustic tiles with vinyl faced tiles.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed the facility full of unnecessary items. Remove everything from the facility that is not necessary to running the facility on a daily basis. Clean out all areas including: the basement, the back stock room, the old walk-ins, and all other storage areas. This is to be completed right away to allow for proper storage of needed items and for proper cleaning.

Physical facilities not maintained in good repair. Observed the FRP coming off the ceiling in the flour room. Repair the FRP so that it is tight to the ceiling.

Repeat Physical facilities not maintained in good repair. Observed the flooring throughout the facility in poor repair due to the structural condition of the facility. Facility has shored up the flooring throughout from the basement area and is working on replacing the flooring now. Facility is to shore up all of the remaining squishy floor areas. Facility is to continue to work on the structural issues of the building.

Repeat No protective shielding on lights. Observed unshielded lights throughout the facility. Shield all lights in the facility or provide plastic coated bulbs.

Facility is not being maintained and must start making improvements right away. A reinspection will be done in 2 weeks. If the facility does not show improvments at the reinspection they will be required to appear at a summons at the health department.
